摘要
目的:探讨老年结直肠癌(CRC)的危险因素及与循环胰岛素样生长因子-1(IGF-1)水平的相关性。方法:选取209例经结肠镜病理确诊的CRC患者为病例组,同期进行健康体检的208例非肿瘤患者为对照组,收集所有研究对象的基本临床资料,采用化学发光免疫分析法检测受检人群的循环IGF-1水平。结果:CRC组吸烟、饮酒、高血压、冠心病史者所占比例高于对照组(P〈0.05),血糖(GLU)、白细胞(WBC)、IGF-1也明显高于对照组,而甘油三酯(TG)明显低于正常对照组(P〈0.05)。多因素二元Logistic回归分析显示,性别、吸烟、高血压、冠心病、GLU〉6.10 mmol/L、WBC〉9.5×109/L以及IGF-1〉194.31 ng/ml是CRC的危险因素,且高水平IGF-1可使CRC发生风险增高(OR=2.77,95%CI:1.02~7.49,P〈0.05)。CRC组循环IGF-1水平在肿瘤长径≥5 cm组高于〈5 cm组,在晚期(Ⅲ~IV)组高于早期(I~Ⅱ)组,低分化组高于高分化和中分化组,伴局部淋巴结转移组高于无局部淋巴结转移组(P〈0.05)。结论:循环IGF-1水平与CRC的进展密切相关,戒烟、低糖低脂饮食在一定程度上可预防CRC的发生。
Objective: To explore the risk factors and the correlation with circulating insulin-like growth factor-1 (tGF-1) in senile colorectal cancer (CRC). Methods: Two hundred and nine patients with coloreetal cancer in case group and 208 non-cancer patients in control group were included in this study. The basic clinical data of all subjects were collected. The serum level of IGF-1 was determined by chemiluminescence technology. Results: The proportion of patients with the history of smoking, alcohol, hypertension and coronary heart disease in CRC group was significantly higher than that of the control group (P〈0.05). Blood glucose, white blood cell and circulating IGF-1 level in CRC group were also significantly higher than those of the control group, while triglycerides was lower (P〈0.05). According to analysis of covariance, gender, smoking, hypertension, coronary heart disease history, blood glucose higher than 6.10 mmol/L, white blood cell higher than 9.5×109/L and IGF-1 higher than 194.31 ng/mL were risk factors of CRC. The odds ratio (OR) of patients with higher circulating IGF-1 was 2.77-fold of the lower (95%CI:1.02-7.49, P=0.045). Circulating IGF-1 was significantly higher in patients with CRC with the tumor no less than 5 cm or at advanced stages (stages Ill and IV) or with low differentiation or with regional lymphoid node metastasis than in those with the tumor less than 5 cm or at early stages (stages I and II) or with middle and high differentiation or without regional lymphoid node metastasis (P〈0.05). Conclusion: Circulating IGF-1 may play an important role in the development of CRC. Keeping away from cigarettes, low-sugar fat diet could reduce the risk of CRC to some extent.
