摘要 脑损伤可分为原发性脑损伤与继发性脑损伤。继发性脑损伤后遗症所带来的身体与心灵的创伤是我国目前高医疗负担的一个重要原因之一。机械通气是一把双刃剑,在挽救生命的同时也能带来一些并发症,例如机械通气肺损伤、脑损伤。机械通气运用恰当时,可以治疗重症脑损伤患者,尤其是呼吸中枢受损的患者。然而,使用不恰当时会加重原有的脑损伤或者使无脑损伤的患者造成脑损伤。对近年来机械通气与脑损害之间关系的研究进展作一综述。趋向针对机械通气的最佳模式以及脑保护策略的最佳设置是目前研究的热点。 Brain injury can be divided into primary and secondary brain injury. Body and soul trauma brought by sequelae of secondary brain injury is one of the important reasons of the China's cur- rent high health burden. This article is present the progress of mechanical ventilation with brain injury. Mechanical ventilation is a double-edged sword, which can save people's life but also can bring some complications, such as lung injury, brain injury. It can treat patients with severe brain damage when me- chanical ventilation is appropriately used, especially in patients with respiratory center impaired. However, the inappropriate use of mechanical ventilation will aggravate original brain injury or cause brain injury to the patients who had no brain injury before. Best mode and setting for mechanical ventilation to the op- timal brain protection policy is a hot topic currently.
