摘要
目的探讨在对冠心病患者进行介入治疗后出现心血管迷走反射的原因,并提出相应的防治措施。方法:择取我院于2015年2月至2016年2月进行心脏介入治疗出现心血管迷走反射的26例冠心病患者,进行回顾性分析。结果 26例患者中,有24例患者是在拔管是出现迷走反射,2例为其它时间;PCI疗法(冠状动脉介入)11例,冠脉造影为15例,早期心脏介入17例,近期10例,桡动脉穿刺7例,股动脉19例,在对患者进行多巴胺和阿托品的静脉给药以及快速补液后,患者症状均得到缓解。结论冠心病患者在进行介入治疗的过程中,其出现血管迷走反应主要与其手术时间、手术方法和穿刺的血管有关,及时进行有效的处理能够避免出现严重后果。
Objective To investigate the causes of cardiovascular vagal reaction after interventional treatment of coronary heart disease and related preventive and treatment measures. Methods A retrospective analysis was performed for the clinical data of 26 patients with coronary heart disease who experienced cardiovascular vagal reaction after heart interventional treatment from February 2015 to February 2016. Results Among the 26 patients, 24 experienced vagal reaction during extubation and 2 experienced vagal reaction at other time points. Of all patients, 11 underwent percutaneous coronary intervention（coronary artery intervention） and 15 underwent coronary angiography; 17 underwent early cardiac intervention and 10 underwent late cardiac intervention; 7 underwent radial artery puncture and 19 underwent femoral artery puncture. All the patients achieved remission after intravenous administration of dopamine and atropine and rapid fluid infusion. Conclusion During interventional treatment for patients with coronary heart disease,the occurrence of cardiovascular vagal reaction is mainly associated with time of operation, surgical method, and vessel for puncture, and timely and effective treatment can avoid serious consequences.
