摘要
目的探讨腹腔镜下广泛性子宫切除术治疗早期子宫颈癌的效果及对患者预后的影响。方法选取2011年2月~2016年5月我院收治的早期子宫颈癌患者40例作为研究对象,随机将其分为对照组和观察组,各20例。对照组行开腹子宫切除术,观察组行腹腔镜子宫切除术,比较分析两组患者手术情况、并发症发生情况及预后情况。结果观察组患者的手术时间明显增加（P〈0.05）,而手术出血量、术后肛门排气时间、气术后排尿功能恢复时间均少于对照组,差异有统计学意义（P〈0.05）。观察组手术并发症发生率为60.00%,对照组手术并发症发生率为85.0%,观察组明显低于对照组（P〈0.05）。术后随访5年,观察组患者术后1年生存率、累积5年生存率和累积5年无瘤生存率与对照组比较,差异无统计学意义（P〉0.05）。结论实施腹腔镜下广泛性子宫切除术治疗早期子宫颈癌疗效与传统开腹子宫切除术疗效相当,但术中出血量更少,患者恢复更快,并且对患者预后并无明显影响,因此是一种具有较好临床应用价值的手术方式。
Objective To analyze the curative effect of laparoscopic hysterectomy in the treatment of early cervical cancer and its effect on the prognosis and quality of life of patients with cervical cancer.Methods 40 cases of early stage cervical cancer in our hospital from February 2011 to May 2016 were selected as the research subjects.The patients were randomly divided into the control group and the observation group,20 cases in each group.The control group was treated with open hysterectomy,the observation group was treated by laparoscopic hysterectomy.The operation status,complications and prognosis of the two groups were compared and analyzed.Results Observation group patients at the time of surgery increased significantly（P〈0.05）,and the amount of bleeding in operation,postoperative anal exhaust time,postoperative gas bladder function recovery time was less than that of control group,the difference was significant difforecee（P〈0.05）.The incidence rate of complications in the observation group was 60%,the control group was 85.0%,the incidence of complications was significantly lower than that of control group（P〈0.05）.After 5 years of follow-up,1 year survival rate,cumulative 5 year survival rate and 5 year survival rate of the patients in the observation group were not significantly different from the control group（P〈0.05）.Conclusion The effect of laparoscopic uterine resection surgery in the treatment of early cervical cancer and traditional open uterine resection is quite,but surgery less bleeding,patients recover faster,and on the prognosis of the patients had no obvious effect.Therefore,it is with a good clinical application value of the operation.
