摘要
目的评价经尿道前列腺电切术联合小切口膀胱取石术治疗前列腺增生症合并膀胱结石的临床疗效。方法选择2010年12月~2012年12月我院确诊并收治的80例前列腺增生症并发膀胱结石患者,采用随机数字表法将其分为两组,其中行开放性手术治疗的30例为对照组;行经尿道前列腺电切术联合小切口膀胱取石术的30例为实验组。比较两组结石清除情况、各观察指标,包括IPSS评分、最大尿流率、并发症发生率、术后导尿管留置时间及住院时间等。结果 80例患者手术均成功,结石清除率为100.0%,无残留问题发生;实验组患者的IPSS评分、并发症发生率低于对照组,术后导尿管留置时间及住院时间均明显短于对照组,最大尿流率显著高于对照组,两组比较差异有统计学意义（P〈0.05）。结论经尿道前列腺电切术联合小切口膀胱取石术治疗前列腺增生症合并膀胱结石较开放性手术结石清除率无明显差异,但能明显缓解前列腺增生症状,同时还具有创伤小、恢复快、并发症少等优势,可作为首选治疗方案在临床推广应用。
Objective To evaluate the clinical effect of combination of application of transurethral resection of prostate（TURP） and small-incision cystolithotomy in treatment of patients suffering from benign prostatic hyperplasia（BPH）complicated with bladder calculi.Methods Altogether 80 cases of BHP patients complicated with bladder calculi who were confirmed in the hospital from December 2010 to December 2012 were divided into two groups evenly according to the random number table method,with 30 cases in each group.Patients in control group underwent open surgeries;while,experiment group was treated with the combination of TURP and small-incision cystolithotomy.The stone clearance and various indexes of two groups were observed and compared,including the international prostate symptom score（IPSS）,maximal urinary flow rate,incidence of complications,the postoperative indwelling duration of catheters and length of stay（LOS）.Results All the 80 cases underwent successful surgeries and the stone clearance rate was 100.0% without the occurrence of residue problems.Experiment group showed great superiorities in IPSS,incidence of complications of experiment group was lower than that of control group,the postoperative indwelling duration of catheters and LOS as well as remarkably shorter and maximal urinary flow rate was higher than that of control group,with statistically significant difference between the two groups（P〈0.05）.Conclusion Combination of application of TURP and small-incision cystolithotomy has no significant difference from the open surgeries in the stone clearance rate.But it proves its effects in excellent alleviation of the symptoms of prostate hyperplasia and advantages of fewer traumas,faster recovery and fewer complications,which can be taken as a preferred treatment method in clinical application.
