摘要
[目的]建立适合儿童病人的安全管理模式,以减少各类安全问题的发生,使经外周静脉置入中心静脉导管(PICC)能更好地运用于临床。[方法]运用失效模式和效果分析(FMEA)法评估风险因素,列出所有的失效模式,选择其中长效模式优先风险数值(RPN)最高的6个失效模式并对其影响因素进行分析,制订完善的安全管理措施,建立安全管理模式。[结果]建立安全管理模式后RPN值较前降低,PICC导管的并发症及非计划拔管率较实施前显著下降(P<0.05)。[结论]应用FMEA将各种不安全因素进行提前评估,再根据评估结果建立标准化的安全管理模式,可有效杜绝在PICC风险问题,减少各种并发症及非计划拔管率。
Objective To establish the safety management model for children patients, in order to reduce the occurrence of all kinds of safety problems, so that PICC can be better used in Clinic. Methods： The failure mode and effect analysis（FMEA） method were used to evaluate risk factors ; listing all failure modes, selecting 6 highest failure models in risk priority number（RPN） ,and analyzing the influencing factors to formulate the perfect safety management measures and safety management model. Results： After the establishment of safety management model,the RPN value of the failure models was lower than that of the former, and the complication of PICC catheter and unplanned extubation rate were statistically significant decreased（P 〈0. 05）. Conclusion： By using the FMEA method,all kinds of unsafe factors could be evaluated in advance. According to the evaluation results, the standard safety management model could effectively prevent the risk of PICC, and reduce the com-plication and the unplanned extubation rate.
