摘要
目的分析睑缘炎患者患眼的细菌分布及药敏结果,为临床治疗提供参考依据。方法选取2010年11月-2015年11月医院眼科门诊确诊为前睑缘炎和后睑缘炎的患者各50例,另选同期眼科门诊体检无眼部疾病的正常人群50名为对照组,分别收集结膜囊和睑板腺分泌物,送实验室行病原菌培养及药物敏感性试验,采用SPSS18.0软件对数据进行统计分析。结果前睑缘炎组患者结膜囊与睑板腺分泌物细菌培养阳性率72.0%和85.0%,后睑缘炎组为73.0%、82.0%,两组比较差异无统计学意义,对照组患者培养阳性率均低于前睑缘炎组和后睑缘炎组(P<0.05);病原菌培养以表皮葡萄球菌、金黄色葡萄球菌及棒状杆菌为主;药敏显示:表皮葡萄球菌和金黄色葡萄球菌对庆大霉素、利福平及妥布霉素较敏感,棒状杆菌对庆大霉素的敏感性最高,革兰阴性菌对常用抗菌药物的敏感性均较差。结论在睑缘炎治疗过程中应针对感染病原菌选择敏感性较好的药物,以获得最佳的治疗效果。
OBJECTIVE To investigate the distribution and drug susceptibility of pathogens isolated from patients with blepharitis so as to provide guidance for clinical treatment.METHODS From Nov 2010 to Nov 2015,totally50 patients who were diagnosed with former blepharitis and 50 patients who were diagnosed with later blepharitis in ophthalmology outpatient department were enrolled in the study,meanwhile,50 healthy people without eye disease who received physical examination in ophthalmology outpatient department were chosen as the control group.The conjunctival sac and meibomian gland secretions were collected and submitted to laboratory for culture of pathogens and drug susceptibility testing,and the statistical analysis of data was performed with the use of SPSS18.0software.RESULTS The positive rate of bacterial culture of conjunctival sac secretions was 72.0% in the former blepharitis group,73.0%in the later blepharitis group;the positive rate of bacterial culture of meibomian gland secretions was 85.0%in the former blepharitis group,82.0%in the later blepharitis group,and there was no significant difference between the two groups;the positive rate of culture of the control group was lower than that of the former blepharitis group and the later blepharitis group(P〈0.05).The Staphylococcus epidermidis,Staphylococcus aureus,and Corynebacterium were dominant among the isolated pathogens.The result of the drug susceptibility testing showed that the S.epidermidis and S.aureus were susceptible to gentamicin,rifampicin,and tobramycin.The Corynebacterium strains were most susceptible to gentamicin.The drug susceptibility rates of the gram-negative bacteria to the commonly used antibiotics were relatively low.CONCLUSIONIt is necessary to choose sensitive antibiotics for the treatment of the patients with blepharitis based on the pathogens causing the infections so as to achieve optimal therapeutic effect.
