摘要
目的:分析骨康胶囊辅助外科治疗对老年桡骨远端骨折患者骨折愈合、微循环状态以及骨代谢的影响。方法:选择2010年9月~2015年9月在陕西省洛南县医院骨二科接受治疗的200例老年桡骨远端骨折患者,随机分为观察组及对照组,各100例。对照组接受常规外科治疗,观察组接受骨康胶囊辅助外科治疗。比较两组骨折愈合,术后1周时检测血清中微循环指标、骨代谢指标,术后6个月时行X线检查并测量影像学参数。结果:观察组术后患侧肢体肿胀的消退时间以及骨折断端影像学愈合的时间显著短于对照组(P〈0.05);术后1周时,观察组血清TXB2、Ⅰ型胶原交联羧基末端肽(CTX-Ⅰ)、Ⅱ型胶原交联羧基末端肽(CTX-Ⅱ)、核转录因子-κB受体活化因子(RANK)、核转录因子-κB受体活化因子配体(RANKL)、抗酒石酸酸性磷酸酶5b(TRACP5b)含量以及TXB-2/6-Keto—PGF1α比例显著低于对照组(P〈0.05),6-Keto-PGF1α、碱性磷酸酶(ALP)、骨保护素(OPG)、Ⅰ型前胶原羧基端肽(PICP)、骨钙素(BGP)、1,25二羟维生素D3[1,25(OH)2D3]含量显著高于对照组(P〈0.05)。结论:骨康胶囊辅助外科治疗有助于促进老年桡骨远端骨折患者的骨折愈合且能够改善微循环状态以及骨代谢状态。
Objective: To analyze the effect of Gukang capsule-assisted surgical treatment on fracture healing, microcir- culation and bone metabolism in elder patients with fracture of distal radius. Methods: A total oF 200 cases of elderly patients with fracture of distal radius treated in Orthopedics Department No. 2 of Luonan Hospital of Shaanxi Province from September 2010 to September 2015 were randomly divided into observation group and control group (n = 100). Control group received rou- tine surgical treatment, and observation group underwent Gukang capsule-assisted surgical treatment. Fracture healing was compared between two groups, serum microcirculation and bone metabolism indexes were detected t week after operation, X- ray examination was performed 6 months after operation and imaging parameters were measured. Results: Regression time ofpostoperative affected-side limb swelling and radiographic healing time of broken ends of fractured bone of observation group were significantly shorter than those of control group (P〈0.05) ; 1 week after operation, serum TXB2, type I collage cross- linked-telopeptide (CTX- Ⅰ ), CTX-Ⅱ , RANK, RANKL and TRACPSb levels as well as TXB-2/6-Keto-PGF1α ratio of obser- vation group were significantly lower than those of control group (P 〈 0.05), and 6-Keto-PGFla, alkaline phosphatase (ALP), osteoprotegerin (OPG), carhoxyterrninal propeptide of type Ipmcollagen (PICP), bone gla protein (BGP) and 1,25 (OH)2 Da levels were significantly higher than those of control group (P〈0.05). conclusions: Gukang capsule-assisted surgery helps promote the fracture healing in elderly patients with fracture of distal radius and can improve microcirculation and bone metabolism.
