摘要 目的探讨环磷酰胺对急性百草枯中毒致肺纤维化的治疗作用。方法选取2012年8月至2015年8月于本院接受治疗的80例急性百草枯中毒致肺纤维化患者为研究对象,根据入院单双号将入选患者分为试验组与对照组,每组各40例。对照组患者行常规抢救治疗,试验组患者在对照组治疗基础上加用环磷酰胺治疗。比较两组患者入院治疗第1、2、3周时的存活率,第1周和第2周肺部高分辨率CT(HRCT)评分,肺纤维化发生率及肺纤维化程度。结果两组患者入院治疗第1周和第2周的存活率比较差异无显著性(P>0.05),但试验组患者第3周存活率显著高于对照组(P<0.05)。两组患者入院治疗第2周肺部HRCT评分均高于第1周(P<0.05),试验组患者入院治疗第2周肺部HRCT评分显著高于对照组(P<0.05)。试验组患者肺纤维化发生率及肺纤维化程度均显著低于对照组(P<0.05)。结论环磷酰胺对急性百草枯中毒致肺纤维化的治疗效果显著,患者存活率高,显著降低肺纤维化程度,保护肺免于被损伤。 Objective To explore the therapeutical effect of cyclophosphamide on pulmonary fibrosis induced by acute paraquat poisoning. Method 80 cases of patients with pulmonary fibrosis induced by acute paraquat poisoning in our hospital from August 2012 to August 2015 were selected as the research objects and according to the odd and even numbers of hospital, they were divided into experimental group and control group, 40 cases in each group. Control group patients were treated with routine rescue treatment, experimental group patients were treated with cyclophosphamide on the basis of control group. The survival rates in the two groups of patients at the first, second and third week, HRCT scores at the first and second week and the incidences of pulmonary fibrosis and the degree of pulmonary fibrosis of the two groups were observed and compared. Result The survival rates of the two groups were not statistically different at the first week and second week(P〈0.05), but the survival rate of the patients in experimental group was much higher than control group at the third week(P〈0.05). The HRCT scores in two groups at the second week were higher than the first week(P〈0.05), while the HRCT scores at the second group in experimental group was significantly higher than control group(P〈0.05). The incidence of pulmonary fibrosis and the degree of pulmonary fibrosis of experiment group were significantly lower than control group(P〈0.05). Conclusion The effect of cyclophosphamide on pulmonary fibrosis caused by acute paraquat poisoning is significant, the survival rate is higher, and significantly decreases the degree of pulmonary fibrosis, to achieve the purpose of protecting from lung injury.
