摘要 目的 :比较配置宽度不同的多叶光栅叶片的加速器对鼻咽癌调强放疗计划的影响,探讨最适合鼻咽癌调强放疗的加速器。方法:选择32例鼻咽癌患者进行调强放疗,利用Pinnacle3 V9.2计划系统分别选择Elekta Precise和Varian 6EX 2台加速器进行调强放射治疗(intensity-modulated radiation therapy,IMRT)计划设计,根据剂量体积直方图对靶区剂量以及危及器官进行综合分析,比较2种计划在靶区适形度和剂量均匀性以及危及器官受照剂量方面的差异。结果:在2种加速器上完成的调强放疗计划的靶区平均剂量之间差别无统计学意义(P=0.863);Varian 6EX加速器的靶区适形度指数和剂量均匀性指数均优于Elekta Precise(P<0.001,P=0.04)。二者在脑干、脊髓、晶体、视神经、腮腺等器官的剂量均无显著差异(P>0.05)。结论:选择Varian 6EX直线加速器完成调强放疗计划,有较好的靶区适形度和剂量均匀性,更适合鼻咽癌的调强放射治疗。 Objective To compare the effects of the accelerators with different widths of multi-leaf collimators on nasop-haryngeal carcinoma(NPC) IMRT plans, and to explore the optimum accelerator for NPC IMRT. Methods Totally 32 NPC patients were selected to undergo IMRT. IMRT plans were made using Pinnacle3 V9.2 TPS on the basis of Elekta Precise and Varian 6EX respectively. The target area dose and involved organs were analyzed with dose-volume histogram. The CI and DHI of target as well as dose received by OARs were compared. Results There were no significant differences in the mean dose of IMRT plans using different accelerators(P=0.863), while the CI and DHI of plans made by Varian 6EX were better than that by Elekta Precise(P〈0.001, P=0.04). There were no significant differences found in the terms of dose received by the OARs, such as brainstem, spinal cord, crystal, optic nerve, parotid gland(P〈0.05). Conclusion Varian 6EX with optimum CI and DHI IMRT plans is suitable for NPC radiation therapy.
