Expression and significance of ER,PR,and HER2 in breast cancer

摘要 目的 探讨雌激素受体(ER)、孕激素受体(PR)以及人类表皮生长因子受体2(HER2)在乳腺癌中的表达和意义。方法 选择2009年1月~2012年1月我院收治的506例乳腺癌患者进行研究,采用免疫组化法检查患者ER、PR、HER2表达水平。结果 HER2阳性率为41.1%,腋窝淋巴结转移患者HER2阳性率(79.8%)显著高于无淋巴结转移患者(20.1%),差异有统计学意义(P〈0.05)。PR阳性率为41.9%,腋窝淋巴结转移患者PR阳性率(31.5%)显著低于无淋巴结转移患者(47.6%),差异有统计学意义(P〈0.05)。ER阳性率为39.5%,腋窝淋巴结转移患者ER阳性率(29.2%)显著低于无淋巴结转移患者(45.1%),差异有统计学意义(P〈0.05)。HER2表达与ER、PR表达呈显著负相关(P〈0.05)。腋窝淋巴结转移患者不良预后率(25.8%)显著高于无腋窝淋巴结转移患者(9.8%),差异有统计学意义(P〈0.05)。HER2阳性组不良预后率(19.2%)显著高于HER2阴性组(8.1%),差异有统计学意义(P〈0.05)。结论 ER、PR、HER2表达与乳腺癌疾病的发生发展关系密切,可将其作为疾病预后与用药选择的重要依据。 Objective To explore the expression and significance of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2) in breast cancer. Methods 506 patients with breast cancer treat- ed in our hospital from January 2009 to January 2012 were selected and the expression levels of ER, PR, and HER2 were detected by immunohistochemistry. Results The positive rate of HER2 was 41.1%. The positive rate of HER2 in patients with axillary lymphatic metastasis was 79.8%, significantly higher than that in patients without axillary lym- phatic metastasis (20.1%, P〈0.05). The positive rate of PR was 41.9%. The positive rate of PR in patients with axillary lymphatic metastasis was 31.5%, which was significantly lower than that in patients without (47.6%, P〈0.05). The posi- tive rate of ER was 39.5%. The positive rate of ER in patients with axillary lymphatic metastasis was 29.2%, which was significantly lower than that in patients without (45.1%, P〈0.05). The expression of HER2 had a negative relation with the expression of ER and PR(P〈O.05). The poor prognosis rate in patients with axillary lymphatic metastasis was 25.8%, significantly higher than that in those without (9.8%, P〈0.05). The poor prognosis rate was significantly higher in the positive HER2 group(19.2%) than in the negative HER2 group(8.1%, P〈0.05). Conclusion The expression of ER, PR, and HER2 has close correlation with genesis and development of breast cancer, which can be used as important bases for disease prognosis and therapy selection.
