摘要
目的比较血清CA125水平检测辅助阴道彩超和超声造影引导下穿刺活检诊断卵巢肿瘤的准确度。方法收集2012年1月至2015年3月来我院就诊的卵巢肿瘤疑似患者200例,所有患者经阴道彩超检查、超声造影引导下穿刺活检及血清CA125水平检测,最终经肿瘤组织病理检查确诊182例,分析上述各项检查诊断准确度的差异。结果血清CA125联合超声造影引导下穿刺活检和血清CA125联合阴道彩超检查的诊断灵敏度、阳性似然比、诊断正确率分别高于单独超声造影活检和单独阴道彩超检查,且前者的诊断准确率显著高于后者查,差异有统计学意义(P<0.05)。结论血清CA125水平检测联合超声造影引导下穿刺活检可提高卵巢肿瘤诊断的准确度。
Objective To compare the diagnostic accuracy of the serum CA125 level combined with transvaginal color Doppler ultrasound and contrast-enhanced ultrasound respectively in ovarian tumors. Methods A total of 200 patients with suspected ovarian cancer treated in our hospital between Jan 2012 and Mar 2015 were examined with biopsy guided by transvaginal color Doppler ultrasound and contrast-enhanced ultrasound while their serum CA125 levels were detected. 182 cases were eventually confirmed to be ovarian tumors by pathological examinations of tumor tissues. The difference in diagnostic accuracy was compared between the above four examination methods. Results The diagnostic sensitivity, specificity, positive likelihood ratio and diagnostic accuracy of CA125 combined with biopsy guided by transvaginal color Doppler and contrast-enhanced ultrasound were higher than those of single transvaginal color Doppler ultrasound or contrast-enhanced ultrasound. The diagnostic results of serum CAl25 combined with contrast-enhanced ultrasound guided biopsy were significantly better than those of serum CA125 combined with transvaginal color Doppler ultrasound examination（ P〈0.05 ）. Conclusion CA125 level combined with biopsy guided by contrast-enhanced ultrasound can improve the diagnostic accuracy of ovarian tumors.
