摘要
目的评价磷霉素与其他抗菌药物联合对碳青霉烯类抗生素耐药肠杆菌科细菌(CRE)的抗菌效果。方法收集2010年1月至2014年12月浙江大学医学院附属第二医院、杭州市中医院、浙江省人民医院和嘉兴市第二医院非重复分离的233株CRE,琼脂稀释法测定磷霉素、亚胺培南、美罗培南、头孢吡肟、头孢他啶、头孢曲松、头孢哌酮/舒巴坦、哌拉西彬他唑巴坦、环丙沙星、阿米卡星、妥布霉素、多黏菌素B、替加环素13种抗菌药物的MIC,微量棋盘稀释法测定磷霉素与美罗培南、头孢毗肟、头孢他啶、头孢哌酮/舒巴坦、环丙沙星、阿米卡星、妥布霉素、替加环素8种抗菌药物联合对30株CRE菌株的药物敏感性。数据采用)(。检验进行统计学分析,P〈0.05时差异有统计学意义。结果233株CRE菌株对磷霉素总的耐药率为45.1%(105/233),其中克雷伯菌属对磷霉素的耐药率最高(61.9%,73/118),肠杆菌属次之(50%,15/30),黏质沙雷菌耐药率为25%(7/28),大肠埃希菌耐药率达8.2%(4/49);磷霉素与替加环素联合对抗CRE菌株效果最好,76.7%(23/30)的菌株呈现协同作用,磷霉素与氨基糖苷类联合,53.3%(16/30)~70%(21/30)的菌株呈现协同作用,磷霉素与环丙沙星联合仅对30%(9/30)的菌株呈协同作用,而磷霉素与p内酰胺类抗生素联合对0—3.3%(1/30)的菌株呈协同作用;尚未发现磷霉素与其他抗菌药物联合存在拮抗作用。结论磷霉素对碳青霉烯类抗生素耐药肠杆菌科细菌有一定的体外抗菌活性,磷霉素与替加环素和氨基糖苷类抗生素联合均呈现较好效果,可为临床治疗由CRE菌株引起的严重感染提供新思路。
Objective To investigate the antimierobial activity of fosfomycin combined with other antibiotics against carhapenem-resistant enterobacteriaceae (CRE). Methods A total of 233 non-repititive CRE isolates were collected from January 2010 to December 2014 from 4 hospitals, including Second Affiliated Hospital of Zhejiang University School of Medicine, Hangzhou Traditional Chinese Medicine Hospital, Zhejiang Provincial People~ Hospital and Second Hospital of Jiaxing. Antimicrobial susceptibility of fosfomycin, imipenem, meropenem, cefepime, ceftazidime, ceftriaxone, cefperazone-sulbactam, piperacillin-tazobactam, ciprofloxaein, amikacin, tobramycin, polymyxin B and tigecycline were determined by agar dilution method. Synergistic effect between fosfomycin and other antibiotics, including meropenem, cefepime, ceftazidime, cefperazone-sulbactam, ciprofloxacin, amikacin, tobramycin and tigecycline against 30 CRE isolates was determined by chequerboard assay. Chi-Square test was used for statistical analysis, and the difference was statistical significant when P 〈 0. 05. Results An overall 45. 1% (105/233) of 233 CRE isolates were resistant to fosfomycin.
