摘要
目的对比观察临床应用1.8 mm和3.0 mm透明角膜切口白内障超声乳化术后泪膜和眼表变化情况。方法选取2013年9月至2015年2月进行诊治的94例白内障患者,按随机数字表法随机分为A、B两组各47例,A组患者给予1.8 mm透明角膜切口白内障超声乳化术,B组患者行3.0 mm透明角膜切口白内障超声乳化术,对比分析两组患者术后2 d、5 d、10 d、30 d时的泪膜和眼表变化情况。结果两组患者术前主观干燥异物感(DES)、Schirmer试验(SIt)、泪膜破裂时间(BUT)以及角膜荧光素染色积分(CFS)组间比较无显著差异(P>0.05),术后2 d、5 d、10 d时两组患者的DES及SIt均较治疗前明显升高(P<0.05),组间比较A组明显低于B组(P<0.05),术后30 d时A组患者的DES及SIt恢复到术前水平,B组患者仍明显高于术前(P<0.05);术后2 d、5 d、10 d时两组患者的BUT及CFS均较治疗前明显降低(P<0.05),术后30 d时A组患者的BUT及CFS恢复到术前水平,B组患者仍较术前差异明显(P<0.05)。结论相比3.0 mm透明角膜切口,1.8 mm透明角膜切口白内障超声乳化术对患者泪膜及眼表损伤较小,值得推广应用。
Objective To observe the changes of tear film and ocular surface after 1. 8 mm and 3. 0 mm clear corneal incisions for ultrasonic phacoemulsification of cataract. Methods A total of 94 patients with cataract treated in this hospital during September 2013 to February 2015 were selected for this study, and they were randomly divided into two groups, group A and group B, each group with 47 cases. Patients in group A were treated with 1. 8 mm clear comeal incision for ultrasonic phacoemulsification of cataract, and patients in group B were treated with 3.0 mm clear corneal incision for ultrasonic phacoemulsification of cataract. The changes in tear film and ocular surface of patients in these 2 groups were analysed in 2 d, 5 d, 10 d and 30 d after operation. Results The difference in preoperative subjective dry foreign body feeling ( DES) , Schirmer test (Sit), tear film break - up time (BUT) and comeal fluorescein staining points (CFS) between patients of these 2 groups was not significant ( P 〉 0. 05), DES and Sit in patients of these 2 groups in 2 d, 5 d and lOd after the operation were significantly decreased ( P 〈 0. 05) , they in group A were lower than those of group B ( P 〈 0. 05) , while levels of DES and Sit in patients of group A were recovered in 30 d after the operation, they in patients of group B were significantly higher than those before operation ( P 〈 0 . 0 5 ) . BUT and CFS in patients of these 2 groups in 2 d, 5 d and 10 d after the operation were significantly decreased than those before treatment ( P 〈0. 05). While BUT and CFS in patients of group A in 30 d after the operation were recovered to preoperative level, patients in group B still had clear difference when they compared with those before operation ( P 〈 0. 05). Conclusion In comparison with the 3. 0 mm clear corneal incision ,1.8 mm clear corneal incision for ultrasonic phacoemulsification of cataract has less impairment in tear film and ocular surface of patient
