摘要 目的观察右美托咪啶(Dex)在心内直视手术心肺转流前对血液动力学的影响。方法选择40例成人拟施心内直视手术患者,随机分为右美托咪定组(D组)和生理盐水组(N组)。分别经外周静脉泵入右美托咪啶和生理盐水。速度6μg/(kg·h),观察时间10 min(负荷剂量1μg/kg,10 min内完成注入)。分别记录给药前(T_0)、给药后1 min(T_1)、2 min(T_2)、3 min(T_3)、5 min(T_4)、10 min(T_5)的SBP、DBP、HR及SPO_2。结果 2组患者血液动力学稳定,D组中与T_0比较,T4-T5的SBP、HR及SPO_2降低;D组与N组比较有差异。结论 Dex在此种输注方式下可产生明显的可唤醒的镇静效应,伴有SBP、SPO_2降低,HR减慢。但所有SBP、DBP、SPO_2、HR均在安全范围。 Objective To evaluate the effects of dexmedetomidine infusion on hemodynamics before CPB undergoing cardiac surgery. Methods 40 cases of adult patients undergoing open heart surgery were selected, were randomly divided into dexmedetomidine group(D group) and saline group(N group), through peripheral venous pump dexmedetomidine and saline, Speed 6μg/(kg·h), The observation time was 10 min(1 μg/kg loading dose, 10 min). Recorded before drug administration(T_0), 1 minute after ministration(T_1), 2 minute(T_2), 3 minute(T_3), 5 minute(T_4), 10 minute(T_5). Results 2 groups of hemodynamic stability, D group compared with T_0, T_4~T_5, SBP, HR and SPO2 decreased, D group and N group were significantly different. Conclusion Dex infusion in this manner can produce significant sedative effect can wake up, accompanied by SBP, SPO_2 decreased, HR slowing. However, all SBP, DBP, SPO_2, HR are in the safe range.
