摘要
目的分析半侧巨脑畸形胎儿的产前超声声像图特征,以提高对半侧巨脑畸形的认识。方法对1例双胎之一左侧侧脑室扩张的胎儿行产前超声检查,超声诊断为半侧巨脑畸形,行产前MRI检查确诊。结合产后新生儿头颅超声及CT检查结果进行对比分析。结果半侧巨脑畸形胎儿产前超声特征主要为:一侧大脑半球增大,大脑皮质增厚,病变侧侧脑室增宽,脑中线向对侧偏移,并可合并病变同侧小脑半球、脑干增大。结论产前超声诊断半侧巨脑畸形具有可行性,对有怀疑的胎儿结合产前MRI可以确诊。
Objective： To investigate the prenatal diagnosis imaging for characteristics of hemimegalencephaly （HME） . Methods： The fetal was referred to our hospital due to the detection of a large abdominal mass in one of the twins.Prenatal MRI showed findings consistent with HME.The results of postnatal high frequency ultrasound an CT were compared. Results： The asymmetric enlargement of the affected side of cerebral hemisphere and ipsilateralventricle, straightening, widening, and flattening of the sylvian fissure on the hypertrophied side were found by prenatal ultrasound combined with the affected side cerebellum and brainstem enlargement, displacing the mindline structures to the opposite side. Conclusions： The diagnosis of HME is possible at the time of the routine anomaly scan.The use of MRI may also be helpful to confirm the diagnosis.
