摘要
目的观察小切口离断式肾盂成形术在肾盂输尿管连接部梗阻(UPJO)患儿中的临床应用效果。方法选取80例UPJO患儿随机分成观察组(40例)和对照组(40例)。对照组予以开放式肾盂成形术,观察组采用后腹腔镜小切口离断式肾盂成形术。比对两组患儿平均术程、术中失血量、术后胃肠道功能恢复时间、总住院时间、止痛药用量等围术期指标差异,记录其术后并发症发生及肾积水复发情况。结果 1两组患儿手术均成功,观察组无中转开放术式报告;对照组平均术程、术中失血量、术后胃肠道功能恢复时间、总住院时间、止痛药用量等围术期指标均显著高于观察组,差异具有统计学意义(P<0.05)。2两组患儿术后肾积水复发率对比无统计学意义(P>0.05);观察组术后并发症发生率为7.5%,显著低于对照组的25.0%(P<0.05)。结论 UPJO患儿应用后腹腔镜小切口离断式肾盂成形术,疗效确切,具有术程短、失血量少、术后恢复快、并发症发生率低等优势,值得临床推广。
Objective To explore the clinical application value of Anderson-Hynes dismembered pyeloplasty with small incision in children with ureteropelvic junction obstruction （ UPJO） . Methods 80 UPJO children were selected as the research objects. They were divided by random number table into study group （ group A , n = 40） and control group （ group B , n = 40） . Group B received open nephropy- eloplasty,group A received retroperitoneoscopic Anderson-Hynes dismembered pyeloplasty with small incision. Differences in average operative course, intraoperative blood loss, recovery time of gastrointestinal function after operation, total length of stay, dosage of analgesics and other perioperative indexes in the two groups were compared. The occurrence of postoperative complications and recurrence of uronephrosis were recorded. Results （1 ）Children in the two groups had successful operation. There was no transfer open surgery report in group A; average operative course, intraoperative blood loss, recovery time of gastrointestinal function after operation, total length of stay, dosage of analgesics and other perioperative indexes in group B were significantly higher than group A （P 〈0. 05） . （2）There was no significant difference in postoperative recurrence rate of uronephrosis in the two groups （ P 〉 0. 05 ） . Postoperative complication rate in group A was 7.5%, which was significantly lower than group B （25.0%） （ P 〈0. 05）. Conclusion Efficacy of retroperitoneoscopic Anderson-Hynes dismembered pyeloplasty with small incision for UPJO children is reliable, and has the advantages of shorter operative course, less blood loss, fast postoperative recovery, low complication rate. It is worthy to be popularized.
