摘要
目的:探讨无痛结肠镜下治疗结肠息肉电切除手术的安全性和临床疗效。方法:选取我院收治的1 171例结肠息肉患者为研究对象,其中585例行常规结肠镜下治疗,观察组586例行无痛结肠镜下治疗,比较两组治疗前后SPO2、DBP、SBP及HR变化、诊治时间和术后恢复情况。结果:观察组插至回盲部时间、手术时间均明显短于对照组(P<0.05);观察组治疗后DBP、SBP及HR值同检查前相比均有所下降,差异有统计学意义(P<0.05);对照组SPO2、DBP、SBP均较检查前略有升高,HR较检查前略有下降,但差异无统计学意义(P>0.05);观察组术后肠功能恢复比对照组快,差异有统计学意义(P<0.05);观察组住院时间明显少于对照组,差异有统计学意义(P<0.05);所有患者手术过程中均未出现肠穿孔,术后少数病人出现穿孔,出血等并发症,积极处理均好转恢复,未出现严重并发症。结论:无痛结肠镜下治疗结肠息肉电切除手术安全可靠,具有显著的临床疗效,能够有效地缩短手术时间,减轻不良反应,加快术后恢复。
Objective： To explore the safety and clinical effects of painless colonoscopic polypectomy for treatment of colon polyps. Methods： We selected 1171 patients with colon polyps admitted to our hospital and divided them into 2 groups： control group（ 585 cases） and observation group（ 586 cases）. Patients in the control group were given routine colonoscopic polypectomy,while patients in the observation group were given painless colonoscopic polypectomy. The SPO2,DBP,SBP,HR before and after treatment,the diagnostic and therapeutic time and the recovery of patients were compared between the two groups. Results： The time of intubating to ileocecus and the operation time in the observation group were significantly shorter than those in the control group（ P〈0. 05）.The levels of SPO2,DBP,SBP and HR of patients after operation in the observation group were significantly lower than that before operation（ P〈0. 05）,the levels of DBP,SBP,and HR of patients after operation in the control group were a little higher,and the levels of HR were a little lower than before operation,but the difference had no statistically significant difference（ P〈0. 05）. The recovery time of intestinal function（ 21. 4 ± 4. 5h） and the time of hospital stay（ 5. 3 ± 1. 2 d） of patients after operation in the observation group were significantly shorter than those（ 35. 6 ± 6. 5 h and 10. 3 ± 2. 5 d） in the control group（ P〈0. 05）. No intestinal perforation occurred in all patients during operation,and intestinal perforation and / or haemorrhage after operation occurred in few patients who recovered soon after time and positive treatment,and no severe complications occurred. Conclusion： Painless colonoscopic polypectomy is a safe and reliable treatment for colon polyps with significant therapeutic effect,can effectively shorten the time of operation,reduce the incidence of adverse reactions and promote the recovery of patients.
