摘要
目的:探讨恩度联合力扑素同步放疗对中晚期宫颈癌患者血清细胞角蛋白19片段抗原21-1(cytokerantin-19-fragment antigen 21-1,CYFRA21-1)、癌胚抗原(carcinoembryonic antigen,CEA)、鳞状细胞癌相关抗原(squamous cell carcinoma antigen,SCCA)、糖类抗原125(cancer antigen 125,CA125)、白细胞介素-8(interleukin-8,IL-8)及免疫细胞水平的影响。方法:选择2014年1月~2016年2月在我院确诊的72例中晚期宫颈癌患者,随机分为实验组(n=36)和对照组(n=36),对照组单纯给予放射治疗,实验组给予恩度联合力扑素同步放疗,3周为1个疗程,两组均接受治疗4个疗程,检测并比较两组治疗前后血清CYFRA21-1、CEA、SCCA、CA125及IL-8水平及外周血CD3^+、CD4^+、CD8^+免疫细胞所占的比值。结果:治疗前,两组血清CYFRA21-1、CEA、SCCA、CA125及IL-8水平比较差异无统计学意义(P>0.05);治疗后,两组血清CYFRA21-1、CEA、SCCA、CA125及IL-8水平均明显低于治疗前,同时实验组以上各血清指标均明显低于对照组,差异均具有统计学意义(P<0.05)。治疗前,两组外周血CD3^+、CD4^+及CD8^+细胞所占比值比较差异无统计学意义(P>0.05);治疗后,两组外周血CD3^+、CD4^+细胞所占比值明显低于治疗前,CD8^+细胞所占比值明显高于治疗前,同时实验组各T淋巴细胞亚群所占比值改变优于对照组,差异均具有统计学意义(P<0.05)。结论:恩度联合力扑素同步放疗能明显降低中晚期宫颈癌患者血清CYFRA21-1、CEA、SCCA、CA125及IL-8水平,改善T淋巴细胞亚群水平,提高患者免疫力,值得在临床上推广应用。
Objective： To study the effect of endostar combined with paclitaxel liposome and radiotherapy simultaneously on serum cytokerantin-19-fragment antigen 21-1（ CYFRA21-1）,carcinoembryonic antigen（ CEA）,squamous cell carcinoma antigen（ SCCA）,cancer antigen 125（ CA125）,interleukin-8（ IL-8） and T lymphocyte subsets in patients with advanced cervical cancer.Methods： A total of 72 patients with advanced cervical cancer in our hospital from January 2014 to February 2016 were enrolled in this study. The subjects were divided into control group（ n = 36） and experiment group（ n = 36） randomly. The control group were treated with radiotherapy,while the experiment group were treated with endostar combined with paclitaxel liposome and radiotherapy simultaneously. Three weeks was a period of treatment and the two groups were treated for 4 periods. The serum CYFRA21-1,CEA,SCCA,CA125,IL-8 levels and peripheral blood CD3~＋,CD4~＋and CD8~＋cells of the two groups before and after treatment were compared.Results： There were no significant differences of the serum CYFRA21-1,CEA,SCCA,CA125,IL-8 levels and peripheral blood CD3~＋,CD4~＋and CD8~＋cells of the two groups before treatment（ P〈0. 05）. The serum CYFRA21-1,CEA,SCCA,CA125 and IL-8 levels of the two groups after treatment were significantly lower than that before treatment（ P〈0. 05）,and those levels of experiment group were significantly lower than control group（ P〉0. 05）. The peripheral blood CD3~＋,CD4~＋cells of the two groups after treatment were significantly lower than that before treatment（ P〈0. 05）; CD8~＋cells of the two groups after treatment were significantly higher than that before treatment（ P〉0. 05）; the ratio change of T lymphocyte subsets in experiment group were significantly better than control group（ P〈0. 05）.
