摘要
目的观察喘可治注射液不同给药方式治疗GOLD分级2-3级稳定期COPD的临床疗效。方法将90例GOLD分级2-3级稳定期COPD患者随机分为3组,治疗A组、治疗B组与对照组,每组30例。对照组予噻托溴铵粉吸入剂治疗,治疗A组予喘可治注射液臀大肌肌肉注射,治疗B组予喘可治注射液双侧足三里穴位注射。各组疗程均为6个月,观察比较症状CAT评分,6 min步行距离、肺功能(FEV_1、FEV_1/FVC、FEV_1%)及中医证候积分的变化情况。结果①组间治疗后比较,治疗A组、治疗B组分别与对照组的CAT评分、6MWD水平差异有统计学意义(P<0.05);治疗B组CAT评分下降、6MWD水平提高的程度均更甚于治疗A组(P<0.05)。②组间治疗后比较,治疗A组、治疗B组分别与对照组的FEV_1、FEV_1/FVC、FEV_1%水平差异有统计学意义(P<0.05);治疗B组FEV_1、FEV_1/FVC、FEV_1%水平升高的幅度均大于治疗A组(P<0.05)。③组间治疗后比较,治疗A组、治疗B组分别与对照组中医证候积分水平差异有统计学意义(P<0.05);治疗B组中医证候积分水平降低的幅度大于治疗A组(P<0.05)。结论喘可治注射液联合噻托溴铵粉吸入剂治疗GOLD分级2-3级稳定期COPD的疗效满意,且喘可治穴位注射比肌肉注射的疗效更优。
Objective To observe the effects of Chuankezhi injection on the treatment of COPD patients in GOLD Ⅱ-Ⅲ stable period via different administration. Methods 90 cases of COPD patients in stable stage of grade 2-3 according to GOLD ranks were randomly divided into 3 groups, treatment group A, treatment group B and control group, 30 cases in each group. Patients in control group were treated with thiazide tiotropium powder for inhalation therapy, treatment group A was treated with Chuankezhi injection for muscle injection at glutens maximus,and treatment group B was treated with Chuankezhi injection for acupoint injection at Zusanli. The course of treatment was 6 months. The changes of CAT score for syndrome, walking distance within 6 min（6MWD）, lung function （ FEVI , FEVI/FVC, FEV1% ） and TCM syndrome score were observed and compared. Results ①Compared with the control group, the differences of CAT score and level of 6MWD in the treatment group A and B were statistical significant （P 〈 0.05 ）. Compared with the treatment grnup A,the CAT score in treatrnenl group B decreased significantly and the level of 6MWD improved more obviously（P 〈 0.05）. ②Compared with the control group, the differences of level of FEVI , FEVI/ FVC,FEV1% in the treatment group A and B were statistical significant （P 〈 0. 05 ）. Compared with the treatment group A, the Jevel of these indicators in treatment group B inereased significantly（P 〈 0.05 ）. ③Compared with the control group, the differences of TCM syndrome score in the treatment group A and B were statistical significant （P 〈 0.05 ）. Compared with the treatment group A, the TCM syndrome score in treatment group B decreased significantly（ P 〈 0. 05）. Conclusion Chuankezhi injection combined with thiazide tiotropium powder inhalant could be used tor the treatment of COPD patients in GOLD Ⅱ-Ⅲstable period, and aeupoint injection showed better efficacy than intramuscular injection.
