摘要
电子病历系统的实施,给医务人员带来了一场信息技术引起的组织变革。其使用意愿受到变革抵制因素和技术促进因素双向影响。基于组织变革理论和技术接受模型,分析了技术组织变革的抵制因素和促进因素对使用意愿的双向影响,并建立了医务人员对电子病历系统使用意愿的研究模型。通过调查医院中使用电子病历系统的医护人员,收集332份有效问卷,采用SPSS和AMOS软件对模型进行验证。研究结果表明:威胁感知、不平等感知负向影响使用意愿,并受个人特征(计算机焦虑和自我效能)的调节;通过加入抵制因素和个人特征因素,使用意愿模型的解释率比技术接受模型大幅提高(66.7%)。研究结果剖析了变革抵制因素和技术促进因素对使用意愿的共同影响,为电子病历系统在我国医院的推广实施提供了理论依据。
The implementation of electronic medical record(EMR)system brings technology-driven organizational change.Based on the theory framework of organizational change and TAM,we analyze the impact of inhibitors and enablers of organizational change on the use of EMR,and test the model in the context of medical staff usage of EMR using survey data of 332 doctors and nurses from a public hospital in Shanghai.SPSS and AMOS are used for data analysis.The results show that organizational change may cause perceived threat and perceived inequity,which negatively influence the intention for use;while personal characteristics such as computer anxiety and self-efficacy moderate the relationship.By adding the variables of inhibitors and personal characteristics,the variance explained by the model increased to66.7%.In conclusion,the study provides a theoretical base for the implementation and promotion of EMR in public hospitals in China.
