摘要
目的探讨全髋关节置换(THA)术后患者满意度及影响患者满意度的因素,分析患者总体满意度与特定方面满意度的关系,以及患者自我报告结局估量(PROMS)能否有效反映患者术后满意度。方法回顾性队列研究自2013-12—2014-07行THA术的207例患者。研究总体满意度、特定方面满意度、WOMAC评分等。结果手术改善患者日常活动能力的效果、患者文化程度、术前期望值的实现程度、医护人员的态度、术前对科室医疗水平的认可程度等因素对患者的满意度有较大影响,通过以上影响因素构建的回归模型可以有效预测82.7%的患者术后的总体满意度,而以WOMAC评分为代表的PROMS对患者总体满意度影响不大,说明PROMS并不能有效反映患者术后总体满意度,而性别、年龄、住院日、并存病等并不能帮助解释患者总体满意度之间的差别。结论要提高THA术后的满意度,良好的手术效果是根本,而与患者做好沟通工作,提高医护人员的服务意识,树立良好的医院口碑则是加分项。
Objective To investigate the factors which influence patients' satisfaction after THA and to explore the relationship between overall satisfaction, satisfaction with specific facts and measured clinical outcomes (patient reported outcome measures, PROMS). Methods The retrospective cohort study was conducted on 207 patients undergoing THA from December 2013 to July 2014. Overall patient satisfaction, WOMAC and satisfaction with specific aspects of surgical outcome were analyzed. Results The overall satisfaction was predicted by: capacity for daily action after surgery; patients' education level; meeting preoperative expectations; medical workers' service attitude; preoperative approve to hospital's medical level. These five factors contributed to a model able to correctly predict 82.7% of the Variation in overall patient satisfaction response. The WOMAC scores carried little weight in the algorithm. Various factors such as age, gender, comorbidities and length of hospital stay did not help explain variation in overall patient satisfaction. Conclusion The good results of surgical treatment are the guarantee of good patient satisfaction. Good communication with patient, good service attitude and good reputation of hospital can contribute to better overall satisfaction.
