尿蛋白对糖尿病人群心脑血管事件及全因死亡的影响 被引量:5

Impact of albuminuria on the risk of cardiovascular diseases and all-cause mortality in diabetic patients

摘要 目的 探讨糖尿病蛋白尿对心脑血管事件及全因死亡的影响。方法 选取2006年7月至2007年10月开滦集团职工健康体检人群。采用尿常规试纸检测晨尿。心脑血管事件及全因死亡随访至2013年12月,应用COX比例风险模型评估发病风险。结果 共有8297例糖尿病患者入选,随访期间共发生心脑血管事件683例,全因死亡事件766例。校正传统危险因素后,多因素分析显示,与蛋白尿阴性者比较,微量蛋白尿(±/+)、大量蛋白尿(≥2+)发生心脑血管事件风险HR分别为1.02(95%CI:0.79~1.30,P〉0.05)和1.34(95%CI:1.07~1.68,P〈0.01),发生全因死亡的风险HR分别为1.51(95%CI:1.21~1.87,P〈0.01)和2.52(95%CI:2.09~3.04,P〈0.01)。结论 尿常规试纸法检测的蛋白尿是糖尿病患者全因死亡的独立危险因素。 Objective To evaluate the relationship between albuminuria and cardiovascular diseases and all-cause mortality among diabetic patients. Methods Kailuan clique employees who were confirmed with diabetes during their medical examination from 2006 to 2007 were included in this prospective cohort study. Albuminuria was measured by urine dipstick test. Cardiovascular diseases and all-cause mortality were recorded until Dec 31,2013. COX proportional hazard regression model was used to evaluate the association between exposures and outcomes. Results A total of 8297 participants in Kailuan Study were included in the analysis. There were 683 cardiocerebral vascular events and 766 deaths during the follow-up. After adjustment for other traditional cardiovascular risk factors, compared with negative proteinuria, the risk of cardiovascular and cerebrovascular events in microalbuminuria (±/+) and macroalbuminuria (≥2 +) was HR 1.02 (95% CI 0. 79 - 1.30,P〉0.05) and 1.34 (95% CI 1.07 1.68,P〈0.01) ,the risk for all-cause mortality was HR 1.51 (95G CI 1.21 - 1.87,P〈0.01) and 2.52 (95% CI 2.09 - 3.04, P〈0.01), respectively. Conclusion Albuminuria measured by urine dipstick test is an independent risk factor for all-cause mortality.
