摘要
目的:分析D-二聚体和肿瘤坏死因子-α(TNF-α)联合检测对2型糖尿病肾病(DN)早期的诊断价值。方法:选择120例2型糖尿病患者,根据尿白蛋白排泄率(UAER)的平均水平,将其分为3组,其中单纯糖尿病组(SDM组,UAER<30 mg/d)36例、早期糖尿病肾病组(EDN组)40例和糖尿病肾病组(CDN组)44例,同时选取40名健康者作为健康对照组。用东芝TBA-120FR全自动生化仪进行检测分析,以酶联免疫吸附法分别测定各组中血清D-二聚体和TNF-α含量。结果:各组组间血清D-二聚体和TNF-α水平相比,差异有统计学意义(t=2.58,P<0.01),且随着病情的进展,血清D-二聚体和TNF-α含量逐渐增高,以CDN组血清D-二聚体和TNF-α含量为最高。结论:联合检测血清D-二聚体和TNF-α水平对2型糖尿病肾病的早期检出有重要诊断价值。
Objective: To analyze the diagnostic value of combined detection of D-dimer and tumor necrosis factor alpha(tumor necrosis factor-α, TNF-α) in early diagnosis of type 2 diabetic nephropathy(DN). Methods: 120 cases of patients with type 2 diabetes were selected according to urinary albumin excretion rate(UAER) and were divided into three groups, including 36 cases of diabetes without nephropathy(SDM group), 40 cases with early diabetic nephropathy(EDN group) and 44 cases with diabetic nephropathy(CDN group). At the same time, 40 cases of healthy persons were taken as the control group. Toshiba TBA-120 FR automatic biochemical analyzer was used in analysis and detection, and the concentration of serum D-dimer and TNF-a was measured by enzyme linked immunosorbent assay(ELISA) in each group. Results: There were significant differences between the two groups(t=2.58, P〈0.01). With the progress of the disease, the concentration of serum D-dimer and TNF-α gradually increased(P〈0.05), and the highest is the concentration of serum D-dimer and TNF-α in the CDN group. Conclusion: There is an important diagnostic value for early detection of type 2 diabetic nephropathy with combined detection in serum D-dimer and TNF-α.
