摘要
目的分析卵巢纤维卵泡膜细胞瘤的CT影像特点和病理对照分析,提高其术前诊断水平。方法回顾性分析经手术病理证实的26例28个卵巢纤维卵泡膜细胞瘤的CT影像资料,分析其特点和病理对照。结果肿瘤形态呈类圆形、卯圆形20例22个,不规则、分叶状6例,边缘清楚者25例,1例蒂扭转边缘模糊;肿瘤最大径在3~15cm,长径平均为(9.2±0.2)cm;平扫CT值12—50HU,平均(29.1±2.64)HU,等低于子宫肌肉的密度,增强后26例病灶均为轻度强化或不强化,CT值增加(2~10)HU,最大强化幅度均〈10HU,平均约(5±0.5)HU,与平扫比较其CT值变化有统计学意义(t=3.539,P=0.006)。2例增强后肿瘤内可见纤细血管影,肿瘤实质轻度强化。结论卵巢纤维卵泡膜细胞瘤CT平扫表现为边缘清楚的等低密度肿块(比子宫及肌肉),增强后乏血供的强化模式,就是轻度强化或不强化。这些CT影像特点将有助于提高对卵巢纤维卵泡膜细胞瘤的认识和诊断水平。
Objective To analyze the characteristics of computed tomography (CT) images of ovarian fibrothecoma and to compare them with the pathological features in order to improve the diagnosis. Methods A retrospective analysis was performed using 28 CT images from 26 pathologically confirmed patients with ovarian fibrotheeoma. Features observed on the CT images were compared with the pathological results. Results Twenty-two tumors exhibited round or oval shapes in 20 cases. In 6 cases, the tumors exhibited irregular or lobulated shapes. Among 25 cases, tumors with clear edges were found, while, in one case with torsion, the tumor exhibited blurred edges. The range of maximum tumor diameters was 3 - 15 cm, with an average of (9.2 ± 0.2) cm. The range of unenhanced CT values was 12 -50 HU, with an average of (29.1 ± 2.64) HU, lower than the density of the uterine muscle. After contrast-enhancement, images of lesions in 26 cases exhibited either slight or no enhancement, and the attenuation range of the CT values was 2 - 10 HU, with a maximum enhancement of less than 10 HU, and an average of approximately (5 ± 0.5 ) HU. Compared with the unenhaneed CT scan, the change in CT values was statistically significant ( t = 3. 539, P = 0. 006). In two cases, shadows of thin blood vessels were visible after enhancement and the images showed slight enhancement in tumor parenchyma. Conclusion In unenhanced CT images, ovarian fibrothccoma manifested as low-density masses (compared with the uterus and muscles) with clear edges. After enhancement, the tumors exhibited an enhancement pattern of low blood supply, i. e. , slight or no enhancement. These features of CT images will help improve our understanding and diagnosis of ovarian fibrothecoma.
