经会阴盆底动态超声对产后早期前腔室结构功能的观察 被引量:12

摘要 目的采用盆底超声观察产后早期前盆腔结构功能改变。方法选取20例未育女性作为对照组,147例产后6周的经阴道产初产妇应用经会阴盆底超声在安静状态下及压力状态下(Valsalva动作)观察前盆腔的结构变化;测量膀胱颈距耻骨联合下缘的垂直距离(BSD)、膀胱颈的移动度(BND)和膀胱尿道后角(RA),有无尿道内口漏斗形成,观察计算两组数据的差异。结果在产后早期,前盆腔两组对比有显著差异,对照组:安静状态下BSD:(2.67±0.39)cm,膀胱尿道后角为(106.12±12.89)°,最大Valsalva时,BSD:(1.68±0.69)cm,膀胱尿道后角为RA(110.12±13.26)°,BND为(0.99±0.49)。产后组:安静状态下BSD:(2.39±0.68)cm,膀胱尿道后角为RA(118.32±15.62)°,最大Valsalva时,BSD:(1.20±0.72)cm,膀胱尿道后角为(128.24±13.63)°,BND为(1.19±0.87);压力性尿失禁(SUI)的发生率是26.53%(39/147),前盆腔脱垂的发生率是57.82%(85/147)。结论经会阴盆底超声能观察产后早期前盆腔的结构、功能改变,为盆底康复提供影像支持。 Objective To explore the changes of structure and function of anterior chamber in early postpartum period with dynamic ultrasound via perinenm pelvic floor. Methods Taking 20 healthy nulliparous women as a control group, we selected 147 women to perform ultrasound via pelvic floor at 6 postpartium weeks at resting state and valsalva condition to investigate the altera- tion in structure of anterior pelvic cavity, to measure the perpendicular distance from bladder neck to lower edge of symphysis pubis (BSD) , the bladder neck mobility (BND) , and posterior vesieourethral angle (RA) , as well as to observe whether there was a funnel formation at the internal urethral orifice so as to Calculate the difference between the two groups. Results In early postpartum period, the differences of those aforementioned indices between these two groups were statistically significant. In the control group, at the resting state, BSD was (2.61 ±0.39) cm, RA was( 106.12 ± 12.89)° and at the Valsalva condition, BSD was ( 1.46 ± 0.49) cm, RA was ( 110.12 ±13.26)° and BND was ( 1. 19 ± 0.49). In the postpartum group, at the rest, BSD was (2.39±0.68) cm, RA was (118.32 ±15.62)° and in Valsalva condition, BSD was (1.36 ±0.72) cm, RA was (125.24± 13.63 )° and BND was ( 1.32 ± 0.87 ). The rate of SUI was 26.53% (39/147), the rate of the anterior chamber prolapse was 57.82% (85/147). Conclusion Observation of the structure and function of anterior ehamber in early postpartum period with dynamic ultrasound via perineum pelvic floor can provide imaging support for pelvic floor rehabilitation.
