摘要
目的了解我国老年期痴呆患者的卫生经济现状.方法：分析国内老年期痴呆患者不同的医护照料模式产生的直接和间接经济负担.结果：我国老年期痴呆患者人数已超过900 万,且以每年1.4% 的速度增长.老年期痴呆患者家庭经济负担和社会卫生经济负担均较重,家庭护理费约为1 500 ～ 2 000 元/ 月,1.8 万～ 2.4 万/ 年; 住院患者更需支出9 752 元/ d,家庭支出至少约3 万元/ 年.敬老院内则为1 366 元/ 月,社区约522 元/ 月.老年期痴呆病情越重,总经济损失越高.对于老年期痴呆患者,欧美国家医疗保险和医疗补助占总费用的70%,我国仅有地区小样本量的经济负担调查.结论：我们应借鉴欧美国家老年期痴呆患者医护照料的先进经验,努力完善相关的卫生政策,设立专项研究小组及研究基金,加大基础研究,建立配套的老年期痴呆患者护理模式.
Objective To investigate the health and economic situation of senile dementia patients. Methods： The directand indirect economic costs were analyzed on caring for senile dementia patients in different medical and nursing mode.Results： More than 9 million people had suffered from senile dementia in China, and the proportion was increasing by 1.4%yearly. Senile dementia patients had made both heavy family financial burden and social health burden. The cost of homenursing was about 1 500-2 000 yuan/month, and 18 000-24 000 yuan/year. The hospitalized patients would need to pay 9 752yuan/day, which was about 30 000 yuan per year. On the other side, the cost in nursing home was about 1 366 yuan/month,and that in community was about 522 yuan/month. The more severity of the dementia, the higher of the financial loss was. Themedical insurance and medical aid for senile dementia patients in European and American took about 70% of the total costs,however, only regional small sample survey could provide convincible relative data in our country. Conclusions： Based onthe advanced medical and nursing experiences on caring for senile dementia patients drawn from Europeans and Americans,a more perfect health policy should be made in our country, and research panel and research fund should be set up, the basicresearch should be strengthened, and the matched nursing mode of caring for senile dementia patients should be established.
