摘要
目的:了解近年来屎肠球菌和粪肠球菌临床分离菌株分布特征及耐药情况。方法:回顾性分析2011年1月—2014年12月解放军总医院第一附属医院分离的828株屎肠球菌和370株粪肠球菌临床分布特征及药敏试验结果。结果:屎肠球菌主要来自于尿液、痰液及分泌物,粪肠球菌主要来自于尿液、分泌物及痰液;屎肠球菌主要分离于烧伤科、老年医学科、ICU,粪肠球菌主要分离于烧伤科、骨科、肾内科;屎肠球菌对青霉素G、氨苄西林、呋喃妥因、高浓度庆大霉素和高浓度链霉素的耐药率明显高于粪肠球菌,对四环素的耐药率低于粪肠球菌,屎肠球菌对万古霉素耐药率远高于粪肠球菌。结论:屎肠球菌和粪肠球菌的易感部位、科室分布以及耐药性等方面均存在差异,屎肠球菌总体耐药率明显高于粪肠球菌。
Objective: To assess the characteristics of clinical distribution and drug resistance of Enterococcus faeciumand Enterococcus faecalis in First Hospital Affiliated to the Chinese PLA Genera1 Hospital to provide a reference forthe clinical medication. Methods: Eight hundred and twenty-eight strains of Enterococcus faecium and 370 strains ofEnterococcus faecalis were isolated in First Hospital Affiliated to the Chinese PLA Genera1 Hospital from January 2011 toDecember 2014, their characteristics of clinical distribution and drug sensitivity were analyzed retrospectively. Statisticalanalysis was performed by whonet 5.6. Results: Enterococci faecium were found mostly in urine, sputum and secretionsamples,Enterococci faecalis mostly in urine, secretion and sputum samples、Enterococci faecium were isolated mainlyfrom Department of Burn, Geriatric Medicine and Intensive Care Unit, Enterococci faecalis were isolated mainly fromDepartments of Burn, Orthopaedics and Nephrology.Drug resistance rates of Enterococcus faecium against penicillin G,ampicillin, nitrofurantoin, high-concentration gentamicin and high-concentration streptomycin were significantly higherthan those of Enterococcus faecalis. However, drug resistance rate of Enterococcus faecium against tetracycline was lowerthan that of Enterococcus faecalis. Drug resistance rate of Enterococcus faecium against vancomycin was significantlyhigher than that of Enterococcus faecalis. Conclusions: The susceptible site, distribution departments and drug resistanceof Enterococcus faecium are different from those of Enterococcus faecalis, the overall drug resistance rate of Enterococcusfaecium is significantly higher than that of Enterococcus faecalis.
