摘要
目的:应用血管内超声(IVUS)观察和评价他汀对动脉粥样硬化进展的疗效。方法:选择新西兰大耳白兔,通过高脂饮食喂养20周,在第4周时对兔的右侧颈总动脉进行球囊损伤以诱导兔的动脉粥样硬化斑块形成。16周时28只兔子被随机均分为对照组(不接受药物治疗)和他汀组(接受阿托伐他汀2mg·kg-1·d-1治疗4周),在16周和20周时进行IVUS检查,观察比较两组的斑块体积和重构指数(RI)。20周时行病理检查,比较两组斑块面积和斑块脂质含量。结果:16周时,两组的斑块体积和RI无明显差异(P均>0.05)。与16周时比较,20周时他汀组的斑块体积和RI均无显著变化,而对照组斑块体积[(77.3±15.9)mm3比(88.3±12.1)mm3]和RI[(0.98±0.13)比(1.33±0.09)]显著增大(P<0.05,<0.01);与对照组比较,他汀组20周时的斑块体积[(88.3±12.1)mm3比(77.9±10.1)mm3]和RI[(1.33±0.09)比(0.97±0.12)]显著减小,P<0.05或<0.01。病理结果显示,与对照组比较,他汀组斑块面积[(4.7±1.3)mm2比(2.4±1.3)mm2]和脂质含量[(53±17)%比(31±20)%]显著减小,P均<0.01。结论:他汀类药物可显著延缓动脉粥样硬化进展,减小斑块体积并改善斑块构成。
Objective:To observe and evaluate therapeutic effect of statin on atherosclerosis progression using intravascular ultrasound imaging(IVUS).Methods:New Zealand big ear white rabbits were selected and fed a high cholesterol diet for 20 weeks.On 4th week,all rabbits underwent balloon injury of right carotid artery to induce atherosclerotic plaque formation.On 16 th week,28 rabbits were randomly and equally divided into control group(didn't receive statin therapy)and statin group(received atorvastatin 2mg kg-1 d-1 for four weeks).Rabbits received IVUS on 16 th and 20 th week,then plaque volume and remodeling index(RI)were observed and compared between two groups.Rabbits received pathological examination on 20 th week,then plaque area and lipid concentration of plaque were compared between two groups.Results:On 16 th week,there were no significant difference in plaque volume and RI between two groups(P〈0.05both).Compared with 16 th week,on 20 th week,there were no significant changes in plaque volume and RI in statin group,and significant rise in plaque volume[(77.3±15.9)mm3 vs.(88.3±12.1)mm3]and RI[(0.98±0.13)vs.(1.33±0.09)]in control group(P〈0.05,0.01);compared with control group on 20 th week,there were significant reductions in plaque volume[(88.3±12.1)mm3 vs.(77.9±10.1)mm3]and RI[(1.33±0.09)vs.(0.97±0.12)]in statin group,P〈0.05 or 0.01.Pathological results indicated that compared with control group,there were significant reductions in plaque area[(4.7±1.3)mm2 vs.(2.4±1.3)mm2]and lipid concentration of plaque[(53±17)% vs.(31±20)%]in statin group,P〈0.01 both.Conclusion:Statins can significantly slow down progression of atherosclerosis,reduce plaque volume and improve plaque composition.
