阵发性房颤射频消融术后应用胺碘酮对复发率影响的研究 被引量:1

摘要 目的探讨阵发性心房颤动(房颤)射频消融术后应用胺碘酮对复发率的影响。方法 48例阵发性房颤行射频消融术患者,均术前常规停用抗心律失常药物,术中采用环肺静脉隔离方法 ,术后随机分为胺碘酮组与非抗心律失常药物组,各24例。胺碘酮组口服胺碘酮3个月,其他治疗同非抗心律失常药物组。观察两组的复发情况。结果胺碘酮组复发率:首次治疗1周时为20.8%(5/24),1个月时为12.5%(3/24),3个月时为12.5%(3/24),12个月时为8.3%(2/24)。非抗心律失常药物组复发率:首次治疗1周时为50.0%(12/24),1个月时为37.5%(9/24),3个月时为37.5%(9/24),12个月时为8.3%(2/24)。两组首次治疗1周及治疗1、3个月时复发率比较差异具有统计学意义(P<0.05),治疗12个月时组间复发率比较差异无统计学意义(P>0.05)。结论阵发性房颤射频消融术后应用胺碘酮治疗可影响对3个月内复发率的判断,对12个月时的复发率无影响。 ObjectiveTo investigate influence by amiodarone applied after paroxysmal atrial fibrillation radiofrequency ablation on recurrence rate.Methods A total of 48 paroxysmal atrial fibrillation radiofrequency ablation patients received preoperative withdrawal of conventional antiarrhythmic drug and intraoperative circumferential pulmonary vein isolation. After operation, they were randomly divided into amiodarone group and non-antiarrhythmic drug group, with 24 cases in each group. The amiodarone group received oral administration of amiodarone for 3 months, and its following treatment is the same as the non-antiarrhythmic drug group. Recurrence condition was observed in both groups.Results The amiodarone group had recurrence rates as 20.8% (5/24) in 1 week of treatment, 12.5% (3/24) in 1 month, 12.5% (3/24) in 3 months and 8.3% (2/24) in 12 months. The non-antiarrhythmic drug group had recurrence rates as 50.0% (12/24) in 1 week of treatment, 37.5% (9/24) in 1 month, 37.5% (9/24) in 3 months and 8.3% (2/24) in 12 months. The difference of recurrence rates in 1 week, 1 and 3 months of treatment had statistical significance between the two groups (P〈0.05), while their difference in 12 months of treatment had no statistical significance (P〉0.05).Conclusion Application of amiodarone after paroxysmal atrial fibrillation radiofrequency ablation shows influence on recurrence rate within 3 months of treatment, while it has no effects on recurrence rate in 12 months of treatment.
