摘要
目的比较分析超声乳化术中使用Oasis预装式虹膜扩张器治疗小瞳孔白内障的临床效果。方法选择2013年12月至2015年7月在我院行白内障超声乳化吸出术的患者30例(47眼),A组为小瞳孔白内障组10例(17眼),B组为正常瞳孔白内障组20例(30眼)。A组手术中使用Oasis预装式虹膜扩张器,其余2组手术方法相同。观察2组患者术中并发症及术后患者视力、瞳孔大小及角膜内皮细胞情况。结果手术并发症:6例手术中Oasis预装式虹膜扩张器第一次未能进入导入装置,调整拉钩之后顺利将虹膜扩张器拉入导入装置,2例需将虹膜扩张器从导入装置中取出,通过拉钩拉入导入器。视力:A组术后1个月BCVA 0.88±0.02,B组术后1个月BCVA 0.92±0.02,两组术后视力差异无统计学意义(P>0.05)。术后瞳孔直径:A组术后1个月瞳孔直径(2.67±0.14)mm,B组术后1个月瞳孔直径(2.60±0.11)mm,两组瞳孔直径差异无统计学意义(P>0.05)。角膜内皮细胞计数:A组患者术后1个月角膜内皮细胞计数(2209.59±63.99)mm^(-2);B组患者术后1个月角膜内皮细胞计数(2330.03±51.81)mm^(-2),两组术前术后角膜内皮细胞计数差异均无统计学意义(均为P>0.05)。结论使用Oasis预装式虹膜扩张器治疗小瞳孔白内障,其术后视力、瞳孔大小和角膜内皮细胞计数与正常瞳孔白内障无显著区别。
Objective To evaluate the clinical effects of Oasis iris expander utilization during phacoemulsification. Methods A retrospective analysis of 17 eyes that had O asis iris expander during phacoemulsification（ group A） and 30 eyes without pupil stretch（ group B） in our hospital from D ecember 2013 to July 2015 were performed. The surgical technique for phacoemulsification and IO L implantation was the same in the tw o groups except for the performance of O asis iris expander. D issection of iris adhesion and use O asis iris expander,dilate the pupil to 6 mm for cataract phacoemulsification,removal of iris expander at the end of the surgery in group A.The complications of the surgery,best corrected visual acuity（ BC VA）,pupil size and corneal endothelial cell density of the tw o groups were compared. Results C omplications of the surgery：O asis iris expander could not be pulled to the device in 6 eyes. Adjust the hooks could facilitated the procedure,and the O asis iris expander were taken out in 2 eyes to facilitated the procedure.BC VA： BC VA at postoperative follow-up of 1 month was 0. 88 ± 0. 02 in group A,0. 92 ± 0. 02 in group B. There was no statistical difference betw een the tw o groups（ P〉 0. 05）. Post-surgery pupil size： Post-surgery pupil size at postoperative follow-up of 1 month was（ 2. 67 ± 0. 14） mm in group A,（ 2. 60 ± 0. 11） mm in group B. There was no statistical difference betw een the tw o groups（ P 〉0. 05）. C orneal endothelial cell density： Post-surgery corneal endothelial cell density at postoperative follow-up of 1 month was（ 2209. 59 ± 63. 99） mm（-2）in group A,（ 2330. 03 ±51. 81） mm（-2）in group B. There was no statistical difference betw een the tw o groups（ P 〉0. 05）. Conclusion O asis iris expander during phacoemulsification is not associated with a statistically significant difference in BC VA,pupil size or corneal endothelial cell density postoperatively compared with results in the control group without pupil stretch
