摘要
目的探讨输卵管积水患者运用经阴道三维超声检查的临床价值。方法选取2012年1月-2015年12月我院90例输卵管积水患者进行分组试验，各30例，Ⅰ组、Ⅱ组和Ⅲ组依次实施传统经腹、经阴道二维和三维超声技术检查。同时给予多普勒重建和容积扫描．评估该三种方案的诊断情况。结果90例患者均由造影、手术病理等检查证实，其中输卵管单侧积水者38例，双侧积水者52例。经超声检测，Ⅲ组共29例检出，诊断符合率占96．7％，显著比Ⅰ组的60．0％和Ⅱ组的73．3％高。差异有统计学意义（P〈0．05）；Ⅰ、Ⅱ组间诊断符合率的比较，差异无统计学意义（P〉0．05）。结论经阴道三维超声操作简单、安全，用于输卵管积水的检查，可直观反映其内部空间结构，准确辨别输卵管与周围脏器的关系及积水性质，对于早期发现病情和指导临床治疗均具有重要意义，值得临床推广。
Objective To explore the clinical value of transvaginal three-dimentional ultrasonography in the diagnosis of patients with hydrosalpinx.Methods From January 2012 to December 2015,90 patients with hydrosalpinxin in our hospital were selected for grouping experiments with 30 patients in each group.In group Ⅰ,Ⅱ and Ⅲ ,traditional transabdominal,transvaginal two-dimentional and transvaginal three-dimentional ultrasonographies were implemented in turn. The Doppler reconstruction and volume scanning were provided at the same time,The diagnosis condition of these three regimens was evaluated.Results 90 patients were testified by radiography,surgery and pathology,among them,38 cases with hydrosalpinx in one side,and the rest with bilateral hydrosalpinx.After ultrasonic testing,there were 29 cases detected in group Ⅲ,and diagnostic accordance rate was 96.7%,much higher than 60.0% in group Ⅰ and 73.3% in group Ⅱ, with a statistical difference （P〈0.05）.Comparison of diagnostic accordance rate between group Ⅰ and group Ⅱ ,and the difference had no statistical significance （P〉0.05）.Conclusion Transvaginal three-dimentional ultrasound is simple and safe in performance and can directly reflect inner spatial structure,and accurately differentiate the relationship between salpinx and peripheral viscera as well as the eharaet of hydrops in the examination of hydrosalpinx,which plays an important significance on early detection of disease and guidance of clinical treatment and is worthy of promotion in clinic.
