脑电双频谱指数指导丙泊酚靶控静脉输注用于小儿腹腔镜手术对血流动力学的影响

摘要 目的探讨脑电双频谱指数(BIS)指导丙泊酚靶控静脉输注用于小儿腹腔镜手术对血流动力学的影响。方法 168例行腹腔镜手术患儿,随机分为观察组(实施BIS指导丙泊酚靶控静脉输注)与对照组(静脉注射丙泊酚实施全身麻醉),各84例。观察麻醉效果。结果观察组手术拔管后平均动脉压(MAP)为(10.43±0.31)k Pa、血氧饱和度(Sp O2)为(96.13±0.84)%、心率(HR)为(105.33±12.75)次/min、丙泊酚用量为(1.21±0.43)mg/kg、苏醒时间为(3.27±0.11)min、不良反应发生率为2.38%,均显著优于对照组的(12.94±0.47)k Pa、(97.69±0.17)%、(117.54±14.37)次/min、(1.79±0.86)mg/kg、(6.84±0.25)min、10.71%,差异均具有统计学意义(P<0.05)。结论 BIS指导丙泊酚靶控静脉输注效果确切,可有效改善患儿血流动力学指标,且患儿苏醒更快,不良反应更少,具有较高的安全性,可在临床广泛推广。 Objective To investigate influence by bispectral index(BIS)-guided target-controlled intravenous infusion of propofol on hemodynamics in pediatric laparoscopic surgery. Methods A total of 168 children receiving laparoscopic surgery were randomly divided into experimental group(BIS-guided targetcontrolled intravenous infusion of propofol) and control group(intravenous injection of propofol for general anesthesia), with 84 cases in each group. Their anesthetic effects were observed. Results The observation group had mean arterial pressure(MAP) after extubation as(10.43±0.31) k Pa, oxyhemoglobin saturation(Sp O2) as(96.13±0.84) %, heart rate(HR) as(105.33±12.75) times/min, propofol dosage as(1.21±0.43) mg/kg, recovery time as(3.27±0.11) min, and incidence of adverse reactions as 2.38%, which were all better than(12.94±0.47) k Pa,(97.69±0.17) %,(117.54±14.37) times/min,(1.79±0.86) mg/kg,(6.84±0.25) min and 10.71% in the control group. Theirdifferences all had statistical significance(P0.05). Conclusion BIS-guided target-controlled intravenous infusion of propofol provides precise effect, and it can improve hemodynamic index in children with quick recovery, few adverse reactions and high safety. This method is worth widely clinical promotion.
