摘要
目的:通过多排螺旋CT(MDCT)血管成像对主动脉弓及其分支血管的相关数据进行测量(如直径间距及角度),为主动脉夹层弓部手术提供基础数据参考。方法:收集本院2004年1月到2014年12月就诊的主动脉A型夹层患者的CTA资料,选择420例患者作为研究组。通过Philips DICOM Viewer R2.6L1-SP2软件及自带测量工具测量各血管的直径、间距及角度。结果:420例患者中,标准型占总病例数的96.3%,占变异型总病例数的3.6%。标准型主动脉弓部直径在性别、年龄、病程及有无高血压组有统计学意义(t=-4.753,P<0.05;t=-2.578,P<0.05;t=-5.238,P<0.05;t=1.01,P=0.313),而在是否马方组无统计学意义(t=1.597,P=0.111)。夹层累及分支与否,对IA直径有统计学意义(t=2.041,P<0.05);对LCCA、LSA直径无统计学意义(t=0.076,P=0.941;t=0.239,P=0.811)。夹层病例弓部分支直径(中位数)IA、LCCA及LSA分别为15.7 mm、10.5 mm及12.3 mm。IA-LCCA间距为18.68±4.21 mm;弓部是否扩张,其间距有统计学意义(t=1.018,P=0.309);LCCA-LSA间距为17.80±3.38 mm,弓部是否扩张,其间距无统计学意义(t=0.382,P=0.703)。夹层是否累及IA,其间距无统计学意义(t=-2.000,P=0.845)。夹层是否累及LSA,其间距无统计学意义(t=-0.508,P=0.612)。夹层累及分支与否,对IA、LCCA、LSA与弓主干夹角无统计学意义(t=0.973,P=0.331;t=0.452,P=0.653;t=0.106,P=0.542)。结论:得到国人A型夹层弓部及其分支血管的各参数范围,为主动脉夹层弓部手术提供基础数据参考。
Objective:Our study was aimed to measure related data angiography in aortic arch and its branch vessels through the multi slice spiral CT (MDCT) ,such as the diameter,spacing and angle,to provide the basic reference data for dissecting aortic arch operation. Methods:CTA data of 420 patients with aortic type A dissection in our hospital from January 2004 to December 2014 were selected as study group. Measurement of diameter,spacing and angle of each blood vessel by DICOM Viewer R2.6L1-SP2 Philips software and its own measuring instrument. Results:Among the 420 cases, standard type in the total number of eases was 96.3%, vari- ant type in the total number of cases was 3.6%. The standard type of aortic arch diameter in gender,age,course of disease and hypertension had statistical significance(t = -4. 753,P 〈0. 05;t = -2. 578 P 〈0.05;t = -5. 238,P 〈 0.05;t = 1.01 ,P = 0. 313) ,The standard type of aortic arch diameter in the Marfan had no statistical significance ( t = 1. 597, P = 0. 111 ). Dissection involving the branch or not in IA diameter had statistical significance (t = 2.041, P 〈 0. 05 ) ;Dissection involving the branch or not in LCCA, LSA diameter had no statistical significance ( t = 0. 076, P = 0. 941 ; t = 0. 239, P = 0.811 ). Dissection cases arch branch diameter (median) of IA,LCCA and LSA respectively was 15.7mm, 10.5mm and 12.3mm. IA-LCCA interval was 18.68± 4.21mm ;arch was expanded or not,the spacing was statistically significant( t = 1. 018, P = 0. 309 ) ; LCCA-LSA interval was 17.80 ±3.38mm, arch was expanded or not, the spacing was no statistically significant (t = 0. 382,P = 0. 703 ).
