摘要
目的:分析无创呼吸机联合吸氧在慢性阻塞性肺病(COPD)伴随慢性Ⅱ型呼吸衰竭患者中的应用效果。方法:选取2005年1月~2014年1月间在我院治疗的48例COPD伴随慢性Ⅱ型呼吸衰竭患者,随机分为观察组和对照组各24例,在常规治疗基础上,对照组患者实施鼻导管吸氧,观察组患者实施无创呼吸机机械通气联合吸氧治疗,比较两组患者动脉血气分析结果、肺功能、患者依从性以及不良反应发生率差异。结果:治疗后,两组患者动脉血气分析结果均趋于正常(P〈0.05),但观察组患者更明显(P〈0.05);两组患者肺功能均明显改善(P〈0.05),但观察组患者肺功能明显优于对照组(P〈0.05);差异具有统计学意义。两组患者不良反应发生率及依从性均无统计学差异(P〉0.05)。结论:无创呼吸机联合吸氧可明显改善慢性阻塞性肺病伴慢性Ⅱ型呼吸衰竭患者预后质量。
OBJECTIVE : To observe the clinical effect of non - invasive positive pressure ventilation and reintroducing oxygen on chronic obstructive pulmonary disease combined with type - Ⅱ respiratory failure. METHODS: 48 patients with chronic obstructive pulmonary disease combined with chronic type - Ⅱ respiratory failure in our hospital from January 2005 to January 2014 were selected. They were randomly divided into control group (n =24) and observation group (n =24). On the basis of routine treatment, control group received nasal catheter oxygen therapy and observation group received non - invasive mechanical ventilation combined with rein- troducing oxygen therapy. The arterial blood gas analysis result, pulmonary function, compliance and adverse reaction rate were com- pared. RESULTS : After treatment the arterial blood gas analysis results of the patients in both groups tended to be normal ( P 〈 0. 05 ) , but the ones in observation group were more obviously ( P 〈 0. 05 ). The pulmonary function of the patients in both groups im- proved significantly ( P 〈 0. 05 ), while the one in observation group was better than that in control group ( P 〈 0. 05 ). There were no significant difference in the compliance score and adverse reaction rate in both groups. CONCLUSION: The prognosis quality of the patients with chronic obstructive pulmonary disease combined with chronic type - Ⅱrespiratory failure can be obviously improved by the treatment of non - invasive positive pressure ventilation combined with reintroducing oxygen.
