带状疱疹后遗神经痛的相关发病因素及其预报模型的建立 被引量:5

摘要 目的探讨带状疱疹后遗神经痛(PHN)的相关发病因素及其预报模型建立的应用价值。方法选取2015年4月~2016年2月期间东莞市人民医院门诊和住院确诊治疗的带状疱疹患者244例,依据是否并PHN分为疼痛组(140例)和无痛组(104例),收集患者临床资料,应用单因素分析法和Logistic回归分析法分析PHN相关发病因素及建立其预报模型。结果单因素分析结果显示,PHN的发生与性别、年龄、及发病时情绪状态、疼痛程度有关,差异有统计学意义(P<0.05);Logistic回归分析法结果显示,性别(OR=4.632,P<0.01)、年龄(OR=5.473,P<0.01)、发病时情绪状态(OR=1.279,P<0.01)、疼痛程度(OR=4.231,P<0.01)是PHN发生的独立高危因素,其预报模型公式为P=1/(1+e^(8.312-1.673X1-1.854X2-0.243X4+1.327X7))。结论对女性、高龄、及发病时情绪状态、剧痛的带状疱疹患者应给予更多的关注和重视,以防治PHN的发生。 Objective To explore the pathogenic factors of post herpetic neuralgia(PHN) and the application value of its prediction model. Methods 244 patients who were diagnosed of herpes zoster in the outpatient section or inpatient department of our hospital from April 2015 to February 2016 were divided into pain group(140 cases) and analgesia group(104 cases) according to PHN conditions. The clinical data of patients were collected, the pathogenic factors of PHN were analyzed by single factor analysis and logistic regression analysis methods, and the prediction model of PHN was established. Results The results of single factor analysis showed that the occurrence of PHN was related to gender, age, emotional state at the time of onset, and pain degree, the differences were statistically significant(P 0.05). The results of logistic regression analysis showed that gender(OR=4.632, P 0.01), age(OR=5.473, P 0.01), emotional state at the time of onset(OR=1.279,P 0.01), and pain degree(OR=4.231, P 0.01) were the independent high risk factors of PHN occurrence, which formula of prediction model is P=1/(1+e^(8.312-1.673X1-1.854X2-0.243X4+1.327X7)). Conclusion We should pay more attention to female and elderly patients with herpes zoster, as well as the patients with abnormal emotional state or sharp pain at the time of onset to prevent and control the occurrence of PHN.
