摘要
目的比较宫腔镜下电切术与刮宫术治疗子宫内膜息肉不孕患者的临床效果。方法选取我院从2014年1月~2015年12月入院治疗的150例子宫内膜息肉不孕患者做为研究对象。按照入院顺序随机分为观察组75例与对照组75例,观察组患者应用宫腔镜下电切术治疗,对照组患者应用宫腔镜下刮宫术治疗。比较两组患者的临床疗效、患者围术期临床指标、术后1年内月经量变化及有无复发。比较两组患者内膜组织中TNF-α、IL-6和IL-1β的m RNA的表达。结果两组患者的围术期指标无显著性差异,不具有统计学意义（P〉0.05）,观察组患者术后复发率及术后阴道出血并发症发生率低于对照组患者,观察组患者治疗后妊娠率高于对照组患者,差异具有统计学意义（P〈0.05）。两组患者术前月经量差异无统计学意义（P〉0.05）,观察组患者术后1年内月经量低于对照组患者,差异具有统计学意义（P〈0.05）。观察组（电切术组）患者TNF-α、IL-6和IL-1β的的m RNA水平显著低于对照组（刮宫术组）,差异有统计学意义（P〈0.05）。结论临床应用宫腔镜下电切术治疗子宫内膜息肉不孕症患者,与传统刮宫术有相比,其优势在于显著减少了患者复发率及阴道出血并发症发生率,对患者的生育功能无影响。
Objective To compare the curative effect of electrical resection under hysteroscopy and uterine curettage in treatment of endometrial polyp infertility patients. Methods 150 cases of endometrial polyp infertility patients were selected as the objects, which were cured in our hospital from January 2014 to December 2015. According to the order of admission, they were randomly divided into observation group and control group with 75 cases in each. Patients in observation group were treated with electrical resection under hysteroscopy, and patients in control group were treated with uterine curettage. Clinical efficacy, clinical parameters of patients during perioperative period, menstrual changes in 1 year after operation, recrudescence or not of the two groups were compared. The m RNA expressions of TNF-α, IL-6 and L-1β in endometrial tissue of the patients in the two groups were compared. Results There was no significant difference between the two groups in the perioperative period with no statistical significance（P 0.05）. The rate of postoperative recurrence and incidence rate of postoperative vaginal bleeding of observation group were lower than those of control group. Pregnancy rate after treatment of observation group was higher than that of control group. The differences were statistically significant（P 0.05）. The difference of preoperative menstrual volume of patients in the two groups was not statistically significant（P 0.05）. Menstrual volume in 1 year after operation of patients in observation group was lower than that of control group, and the difference was statistically significant（P0.05）. The m RNA expressions of TNF-α, IL-6 and L-1β of observation group（electrical resection group） were higher than that of control group（curettage group）, and the difference was statistically significant（P 0.05）. Conclusion Compared with the traditional curettage, the advantage of clinical application of electrical resection under hysteroscopy in treatment of endometrial polyp infertility
