摘要
目的探讨普米克令舒吸入与静脉注射甲基泼尼松龙治疗COPD急性发作的临床效果及对患者生活质量的影响。方法选择COPD急性发作患者100例,随机分为观察组与对照组各50例,其中对照组给予甲基泼尼松龙静脉注射治疗,观察组给予普米克令舒吸入治疗,比较2组临床效果、治疗前后血气指标、生活质量量表（SF-36）评分及不良反应发生率。结果观察组治疗后有效率为94.0%明显高于对照组的80.0%（χ^2=7.223,P〈0.05）;观察组治疗后PaCO_2、PaO_2、SaO_2、pH改善优于对照组（t1=12.381,t2=10.326,t3=11.285,t4=10.725,均P〈0.05）;治疗后观察组SF-36各维度评分较对照组均改善明显（P〈0.05）;观察组不良反应发生率为4.0%,明显低于对照组的16.0%（χ^2=8.455,P〈0.05）。结论普米克令舒吸入较静脉注射甲基泼尼松龙治疗COPD急性发作疗效显著,可有效改善患者生活质量,值得临床推广应用。
Objective To investigate the clinical effect of Pulmicort Respules inhalation and intravenous injection of methyl prednisolone in treatment of acute episode of COPD,and its influence on the quality of life of the patients. Methods100 cases of patients with acute episode of COPD were randomly divided into observation group and control group with 50 cases in each group,the control group was given intravenous methylprednisolone for treatment while the observation group was treated with Pulmicort Respules inhalation. The clinical effect,blood gas indexes,quality of life scale（ SF-36） score and incidence of adverse reaction were compared between the 2 groups. Results The efficiency of the observation group after treatment was94%,significantly higher than 80% of the control group,with statistical significance difference（ χ^2= 7. 223,P〈0. 05）; The improvement of PaCO_2,PaO_2,SaO_2,p H of the observation group after treatment was better than the control group,the difference was statistically significant（ t1= 12. 381,t2= 10. 326,t3= 11. 285,t4= 10. 725,all P〈0. 05）; After treatment,each dimension SF-36 scores of the observation group were improved significantly more than the control group,the difference was statistical significance（ P〈0. 05）; The incidence of adverse reaction of the observation group was 4%,significantly lower than 16% of the control group,the difference was statistically significant（ χ^2= 8. 455,P〈0. 05）. Conclusion Pulmicort Respules inhalation in the treatment of acute exacerbations of COPD is significantly more effective than intravenous methyl prednisolone,can effectively improve the quality of life of patients,it is worthy of clinical application.
