瑞格列奈改善新诊断2型糖尿病患者胰岛β细胞功能的研究 被引量:15

Repaglinide improves islet β-cell function in patients with newly-diagnosed type 2 diabetes mellitus

摘要 目的比较瑞格列奈和格列齐特缓释片对新诊断2型糖尿病患者血糖和胰岛β细胞分泌功能的影响。方法 42例新诊断2型糖尿病患者随机分为瑞格列奈组和格列齐特缓释片组,进行为期4周的平行对照研究,两种药物治疗前后测定空腹和标准餐后血糖、血清真胰岛素和胰岛素原水平,用早期胰岛素分泌指数评价早相胰岛素分泌功能。结果瑞格列奈和格列齐特缓释片降低血糖的效果相似(P>0.05);与格列齐特缓释片相比,瑞格列奈促进餐后真胰岛素分泌(P<0.05),并使真胰岛素分泌峰值前移至90 min;瑞格列奈恢复早期胰岛素分泌指数作用显著高于格列齐特缓释片(10.89 vs 6.31,P<0.05);瑞格列奈还显著降低空腹、餐后30、45、60、90、120和180 min的胰岛素原/真胰岛素比值(P<0.05)。结论瑞格列奈可改善新诊断2型糖尿病患者早相胰岛素分泌,降低胰岛素原/真胰岛素比值,改善胰岛β细胞功能。 Objective To determine the effects of repaglinide(Rg)versus gliclazide(Gli)on the hyperglycemia controland insulin secretion of islet β-cells in the patients with newly diagnosed type 2 diabetes mellitus(T2DM). Methods Anopen-labelled parallel trial was conducted on 42 newly diagnosed T2 DM patients treated by Rg or Gli during a period of 4weeks. Plasma glucose(PG),serum true insulin and proinsulin at fasting and after a standard meal were measured before andafter treatment of the 2 agents. Early- phase insulin secretion was assessed using early- phase insulin secretion index.Results Rg and Gli showed similar effects on postprandial hyperglycemia in the subjects(P〉0.05). Compared with Glitreatment,Rg treatment promoted serum true insulin after meal(P〈0.05),and made the time to maximum insulinconcentration occur earlier(90 min). Rg treatment also resulted in a significant increase of early-phase insulin secretion indexwhen compared with the Gli treatment(10.89 vs 6.31,P〈0.05). Fasting,postprandial 30,45,60,90,120 and 180 minproinsulin- to-insulin ratios were decreased significantly in Rg group(P〈0.05). Conclusion Rg treatment promotes earlyinsulin secretion,decreases proinsulin- to- insulin ratios,and improves islet β- cell function in newly- diagnosed T2DMpatients.
