摘要
[目的]观察半夏白术天麻汤＋耳穴压豆联合西药治疗眩晕疗效。[方法]使用随机平行对照方法,将100例门诊患者按就诊顺序号抽签方法简单随机分为两组。对照组50例根据呕吐、恶心、眩晕情况,胞二磷胆碱0.5~1.0g＋5%葡萄糖注射液250m L,静滴;地芬尼多,25mg/次,3次/d,口服;尼莫地平20mg/次,3次/d,口服。治疗组50例半夏白术天麻汤＋耳穴压豆;半夏白术天麻汤（甘草、水蛭各6g,石菖蒲15g,泽泻30g,川芎15g,葛根30g,丹参20g,陈皮15g,茯苓20g,白术、法半夏、天麻各15g）,浸泡1h,水煎200m L,1剂/d,早晚温服;耳穴压豆：用胶布将1粒王不留行籽固定在耳廓相应穴位：额、神门、枕、皮质下、肾上腺等,嘱咐患者用手进行按压刺激,疏通经络,1~2min/次,2~3次/d,2~3更换一次王不留行籽;西药治疗同对照组。连续治疗10d为1疗程。观测临床症状、血栓素B2（TXB2）、血小板活化因子（PAF）、左侧椎动脉（LVA）血流速度、右侧椎动脉（RVA）血流速度、基底动脉（BA）血流速度、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组显效37例,有效12例,无效1例,总有效率98.00%。对照组显效24例,有效9例,无效17例,总有效率66.00%。治疗组疗效优于对照组（P〈0.01）。血栓素B2（TXB2）、血小板活化因子（PAF）及平均血流速度两组均有改善（P〈0.05,P〈0.01）,治疗组改善优于对照组（P〈0.01）。[结论]半夏白术天麻汤＋耳穴压豆联合西药,疗效满意,无严重不良反应,值得推广。
[Objective] To observe banxiabaizhutianma Decoction plus auricular plaster therapy combined with western medicine in treatment of vertigo. [Methods] 100 outpatients were randomly divided into two groups according to the random parallel control method. The control group （50 cases） according to vomiting, nausea, vertigo, citicoline 0.5-1.0g＋5% glucose injection 250 mL, intravenous drip; diphenidol, 25 mg/time, 3 times daily, orally; nimodipine 20 rag, 3/d, oral. 20 g 15 g 15 g 30 g each treatment group of 50 cases of Banxia Baizhu Tianma Decoction and ear pressure beans; Banxia Baizhu Tianma Decoction （Radix Glycyrrhizae, leech 6G, Rhizoma Acori graminei 15 g, Rhizoma alismatis, Rhizoma Chuanxiong, Radix Puerariae 30g, Salvia miltiorrhiza 20 g, dried tangerine peel, Poria, Atractylodes, pinellia tuber, Rhizoma Gastrodiae 15 g）, soak for 1 hour, water simmer in Water 200 ml, 1 dose/day, sooner or later, warm clothes; ear pressure beans： tape will be 1 grain of seed of cowherd seed is fixed in the auricular acupoints： amount, God door, occipital and cortical, adrenal gland, and charged with hand pressing stimulation, dredge the meridians, 1-2 times, 2-3 times daily, 2-3 replacement of a seed of cowherb seed; western medicine with the control group. Continuous treatment for 1 courses of 10 d. Observation of clinical symptoms and thromboxane B2 （TXB2）, platelet activating factor （PAF）, the left vertebral artery （LVA） blood flow velocity, right vertebral artery （RVA） blood flow velocity and the basilar artery （BA） blood flow velocity, adverse reactions. Continuous treatment for 2 courses of treatment, to determine efficacy. [Results] In the treatment group, 37 cases were markedly effective, 12 cases were effective, 1 cases were ineffective, the total effective rate was 98%. In the control group, 24 cases were markedly effective, 9 cases were effective, 17 cases were ineffective, the total effective rate was 66%.
