摘要
目的通过白内障患者术前眼睑热敷、按摩及清洁护理,探讨该护理方法对围手术期结膜囊微生物状态的影响,以寻求更安全的白内障手术术前护理方法。方法 87例90眼拟行白内障手术的患者随机分为A组、B组2组(A组对照组:采用常规术前准备:术前3d抗生素眼液点眼、冲洗泪道及结膜囊、剪眼睫毛等常规术前准备,45例45眼;B组试验组:常规术前准备+睑板腺特殊护理,42例45眼),分别于2个时间点(即手术开始前和手术结束后)采集结膜囊分泌物标本,进行微生物培养。结果结膜囊细菌培养阳性率:A组手术开始及术毕2个时间点的阳性率分别为6.7%、17.8%,B组为2.2%、4.4%。2组组间微生物阳性率比较差异有统计学意义(P<0.05)。结论白内障术前除常规准备外,增加对睑板腺热敷、按摩、清洁等护理,可以降低结膜囊细菌检出率,减少结膜囊二次污染,达到有效清洁术野、减少结膜囊微生物的作用,避免或减少白内障术后眼内炎的风险。
Objectives To explore the influence of the preoperative nursing such as hot compress,massage and cleaning on perioperative mi- crobial status of conjunctival sac on patients with cataract. Methods Patients with the number of 87 patients with 90 eyes were divided ran- domly into 2 groups. The control group called A group including 45 patients with 45 eyes were treated by routing preoperative nursing. The treatment group called B group including 42 patients with 45 eyes were treated by special meibomian gland care and routing preoperative nurs- ing. Collected the samples from secretions of Conjunctival sacfor microbial culture before and after operation. Results The positive rates of the conjunctival sac microbial culture： the control group were 6.7% and 17.8% before and after operation ,the treatment group were 2.2% and 4.4% before and after operation. The positive rates between the 2 groups were different （ P 〈 0.05 ）. Conclusions The preoperative nursing such as hot compress, massage and cleaning could greatly help reduce the eonjunctival sac bacteria detection rate, decrease the secondary pol- lution of conjunetival sac, achieve effective cleaning operation field and lessen the risk of endophthalmitis after cataract surgery.
