摘要
目的分析主动脉弓及弓降部先天性发育异常的影像学表现,提高对该病的诊断水平。方法回顾性分析26例主动脉弓及弓降部先天性发育畸形患者的临床资料。26例患者均拍摄胸部正侧位片,4例行食道或上消化道钡餐透视检查,7例行CT平扫加增强扫描,16排螺旋CT及双源CT者行最大密度(MIP)、容积重建(VR)及多平面重建(MPR)等三维重建。4例主动脉缩窄患者行手术治疗。结果26例患者中,右位主动脉弓15例、双主动脉弓1例、迷走右锁骨下动脉4例,主动脉缩窄6例,其中单纯型主动脉缩窄3例,复杂型3例。结论影像学综合检查在诊断主动脉弓及弓降部先天性发育畸形方面具有全面、可靠、无创的显示主动脉形态学病变的特点,同时可显示其对周围器官的影响,在很大程度上可替代有创性血管造影。
Objective To analyze the imaging performance of congenital developmental abnormality of the aortic arch and its descending part. Methods 26 patients were analyzed retrospectively. 26 patients accepted anteroposterior and lateral chest scan, 4 patients accepted barium meal examination in esophagus or upper digestive tract, and 7 patients accepted CT plain and enhanced scan. For those examinations based on 16-slice spiral CT and dual-source CT, 3D reconstructions including MIP, VR and MPR were conducted. 4 patients with aortic coarctation accepted surgical treatment. Results Among the 26 patients, 15 of them with right-sided aortic arch, 1 of them with doubled aortic arch, 4 of them with aberrant right subclavicular artery, aortic banding in 6 cases, of which 3 cases were simple aortic constriction, 3 cases of complex type. Conclusion Comprehensive imaging examinations can comprehensively, reliably and non-invasively reveal the morphological change of the aorta in the diagnosis of congenital developmental abnormality of the aortic arch and its descending part.
