摘要
目的分析肝肺吸虫病的超声造影特点,提高超声造影对肝肺吸虫病的诊断水平。方法选择2011年6月至2015年12月在四川大学华西医院行超声造影检查并经手术病理或穿刺活组织检查证实为肝肺吸虫病27例患者,其中男性17例,女性10例;年龄24-60岁,平均年龄43.0岁。使用Philips iU22型超声诊断仪进行超声造影,回顾性分析肝肺吸虫病的超声造影特点。结果 27个病灶中14个(51.85%)在动脉期呈高增强,其中8个(29.63%)门静脉期和延迟期均呈低增强,3个(11.11%)门静脉期呈等增强,延迟期呈低增强;3个(11.11%)门静脉期及延迟期均呈等增强;6个病灶(22.22%)在动脉期呈等增强,其中4个(14.81%)门静脉期呈等增强,延迟期呈低增强;2个(7.41%)门静脉期和延迟期均呈等增强。7个病灶(25.93%)呈3个时相低/无增强。不同病灶在动脉期增强形态不同,14个高增强病灶均表现为不均匀增强伴部分坏死,其中9个周边可见环状强化;6个等增强病灶中2个(7.41%)动脉期呈整体均匀增强,4个(14.81%)呈不均匀增强,其中2个周边可见环状强化。病灶内部坏死形态以隧道样改变为主(48.15%)。结论肝肺吸虫病的超声造影表现具有多样性,超声造影能较好地评估肝肺吸虫病的血流灌注特征,提高对肝肺吸虫病的诊断能力。
Objective To analyze the characteristics of contrast-enhanced ultrasound(CEUS) in hepatic paragonimiasis for im-proving the accuracy of diagnosis. Methods From June 2011 to December 2015, a total of 27 patients with hepatic paragonimia-sis examined by CEUS and confirmed by surgical pathology or biopsy were enrolled, which included 17 males and 10 females;aged 24-60 years old with mean age of 43.0 years old. The CEUS examination were performed with Philips i U22 scanner, and characteristics of hepatic paragonimiasis lesions were retrospective analyzed. Results Among 27 lesions, 14(51.85 %) exhibited hyperenhancement in arterial phase. Among these 14 cases, 8(29.63 %) appeared as hypoechoic enhancement in both portal phase and late phase and 3(11.11 %) appeared as isoechoic enhancement in portal phase and hypoechoic enhancement in late phase;3(11.11 %) appeared as isoechoic enhancement in both portal phase and late phases; 6(22.22 %) exhibited isoechoic enhancement in arterial phase, of which 4(14.81 %) appeared as isoechoic enhancement in portal phase and hypoechoic enhancement in late phase and 2(7.41 %) as isoechoic enhancement in both portal phase and late phase. Seven lesions(25.93 %) showed hypoenhance-ment or absent contrast enhancement in 3 phases. The different lesions exhibited different enhancement pattern in arterial phase,14 hyperenhanced lesions showed inhomogeneously enhancement accompanied with partial necrosis, of which 9 appeared as ring enhancement; Six isoenhanced lesions were homogeneously enhanced in 2(7.41 %) and inhomogeneously enhanced in 4(14.81 %),of which 2 presented as ring enhancement. For characteristics of necrotic area, the lesions predominantly exhibited tunnel-like necrotic area(48.15 %). Conclusion It is demonstrated that the enhancement manifestations of hepatic paragonimiasis are di-verse, CEUS is helpful in the diagnosis for hepatic paragonimiasis based on its good perfusion visualization.
