摘要
为了观察后盆腔修补术治疗中老年女性重度直肠前突的临床疗效，回顾性分析2013年3月至2015年6月天津市人民医院肛肠外科，应用后盆腔修补术治疗中老年重度直肠前突120例患者的临床资料，分别于术前和术后3个月对120例患者行排粪造影检查，并采用中华医学会便秘的症状评估表评估便秘改善情况。结果显示，107例患者完成全部随访，患者直肠前突、黏膜脱垂、黏膜内套叠、会阴下降的发生率均较术前显著下降（P〈0．01）。直肠前突程度由术前的（37．5±2．6）mm下降至（4．0±1．3）mm。便秘症状评分由术前的（12．0±4．5）分下降至（2．2±1．0）分，术前患者排便困难、排便不尽感、排便梗阻感等不适症状消失，排便较前明显通畅，平均手术时间（78±16）min，术后住院时间平均6．8d，没有出现感染、血肿、窦道、瘘管形成等严重并发症。结果表明，盆底修补术治疗中老年女性重度直肠前突疗效确切，安全性高，创伤小、恢复快，并发症少，值得临床推广应用。
This study was to observe the clinical effect of posterior pelvic repair for treating mid-senile woman severe rectocele（RC）.The authors analyzed retrospectively clinical data of 120 patients with mid-se- nile severe RC after performing posterior pelvic repair in surgical Dept. of colorectal,Tianjin Union Medi- cine Centre from Mar 2013 to Jun 2015.The 120 patients were performed defecography three months before and after operation and the status of constipation improvement was evaluated by constipation symptom e- valuation sheet of Chinese Medicine Association.Results show that follow-up of 107 patients were finished. In rectocele, mucosal prolapse, mucous membrane intussuseption,perineal descent,occurrence rate of 107 patients was significantly lower than pre-operation （ P 〈0.01）. Degree of RC reduced from （37.5 ± 2.6） mm of pre-operation to （4.0±1.3） mm.Constipation symptom score reduced from （12.0±4.5） of pre-oper- ation to （2.2 ±1.0）. Defecation difficulty, defecation unfinished feeling, defecation obstruction and other pre-operation uncomfortable symptoms were disappeared ＆ Constipation was more smooth than before;The average operation time was （78±16） min; the average hospital stays 6.8d;the severe complications such as infection, hematoma, sinus tract and fistula were not found.In conclusion, the above-mentioned therapy for treating mid-senile woman severe RC has certain effect. And it has such merits as higher safety, minor wound, faster recovery and fewer complications. So it is worthy to be popularized and used clinically.
