摘要
目的探讨慢性牙周炎患者龈下菌斑对种植体周骨吸收的影响。方法选择2013年8月到2016年1月进行种植体修复的后牙缺失患者72例,其中健康对照组40例患者中共植入种植体50颗,而牙周炎组32例共植入种植体40颗。两组都进行常规种植体修复,牙周炎组同时在术前行牙周基础治疗。观察与记录种植前与种植后3个月的龈下菌斑、种植体周骨吸收与龈沟出血情况。结果修复前,牙周炎组的龈下菌斑指数明显高于健康对照组(P<0.05),修复后两组的龈下菌斑指数均呈现下降趋势(P<0.05),但组间对比差异无统计学意义(P>0.05)。修复前的近中与远中边缘骨吸收量之间的对比差异无统计学意义,修复完成后,两组种植体周骨吸收量均有明显减少(P<0.05),且牙周炎组近中边缘骨吸收量降低更明显,远中边缘骨吸收量降低不如对照组明显(P<0.05)。两组修复后的龈沟出血指数都明显低于修复前((P<0.05),且修复后牙周炎组的龈沟出血指数明显高于对照组(P<0.05)。结论慢性牙周炎患者的龈下菌斑比较多见,能影响种植体周骨吸收情况,牙周基础治疗对种植体周围边缘骨的稳定性起促进作用。
Objective To investigate the effect of subgingival plaque of patients with chronic periodontitis of implant peri bone resorption. Methods From August 2013 to January 2016,72 posterior tooth loss patients were selected,including the healthy control group(40 patients with the implants of 50 teeth)and the periodontitis group(32 patients with the implants of 40 teeth). Two groups were given the conventional implantrestoration,and the periodontitis group were given the basic periodontal treatment. The situation of subgingival plaque,bone absorption and sulcus bleeding before planting and after planting 3 months were observed and recorded. Results The subgingival plaque index in the periodontitis group was higher than that in the healthy control group,the difference was significantly( P 〈 0. 05),and the subgingival plaque index in the two groups after planting were significantly decreased( P 〈 0. 05),but the difference between two groups were no statistical significance( P 〉 0. 05). The near and far marginal bone absorption quantity in the two groups before planting were compared and the difference was not statistically significant, which were significantly reduced after planting( P 〈 0. 05),but the near and far marginal bone absorption quantity after planting in the health control group and the periodontitis group were also statistically significant( P 〈 0. 05). The gingival sulcus bleeding index in the two groups after planting were significantly lower than before( P 〈 0. 05),but the gingival sulcus bleeding index in the periodontitis group after planting were sig-nificantly higher than the control group( P 〈 0. 05). Conclusion The subgingival plaque in the patients with chronic periodontitis is common, that can affect the peri implant bone resorption. Periodontal treatment play an important role on the stability of peri implant marginal bone.
