摘要
目的了解新疆某三级甲等综合医院医院感染状况,为预防病原菌感染与有效的控制医院感染提供基础数据,以利于医院感染状况的改善。方法对新疆某三级甲等综合医院2012—2014年58 182例出院病人病历进行医院感染回顾性调查分析。结果 2012—2014年该院检出病原菌阳性15 472例,阳性检出率为26.59%。比较29个科室的阳性检出率,病原菌阳性检出率构成比居于前3位的科室分别为:ICU占16.53%,肾病科占12.13%,呼吸科占11.52%。革兰阴性菌仍然是医院感染的主要病原菌,占74.02%;居前3位的病原菌分别为大肠杆菌(23.50%)、肺炎克雷伯菌(20.80%)、鲍曼不动杆菌(10.62%)。结论应加强对医院病原菌阳性检出率和感染率较高科室及重点病原菌的监测,严格控制抗生素的临床使用,以有效地降低医院感染率与病原菌的耐药率。
Objective To investigate the status of nosocomial infections in a tertiary general hospital in Xinjiang and provide basic data for preventing pathogenic bacteria infection and effective control of nosocomial infections so as to improve the status of the infection. Methods A retrospective analysis was conducted in case history of 58 182 patients discharged from a tertiary general hospital-during 2012—2014 period-regarding nosocomial infections. Results Pathogenic bacteria were detected in 15 472 patients discharged during 2012—2014,the positive detection rate being 26. 59%. A comparison among 29 departments regarding the detection rate,the top three were ICU,department of nephrosis and department of respirology,being 16. 53%,12. 13% and 11. 52%,respectively. Gram-negative bacteria remained dominant pathogens for hospital infections,accounting for 74. 02%,the top three pathogens were Escherichia coli（ 23.50%）,Klebsiella pneumoniae（ 20. 80%）,and Acinetobacter baumannii（ 10. 62%）. Conclusion Monitoring of departments with higher detection rate of pathogens and infection rate as well as key pathogenic bacteria should be enhanced,clinical use of antibiotics be strictly controlled,so as to effectively reduce nosocomial infection rate and the drug resistance rate of pathogenic bacteria.
