肠内营养联合肠道益生菌在肝硬化失代偿期治疗中的价值研究 被引量:9

Value of Enteral Nutrition Combined with Intestinal Probiotics in the Treatment of Liver Cirrhosis in Loss Compensation Period

摘要 目的:探讨肠内营养联合肠道益生菌在肝硬化失代偿期治疗中的应用价值及意义。方法:将2015年1-8月本院收治的肝硬化失代偿期患者64例根据入院时胆红素、白蛋白、凝血酶原时间、血氨等进行Child-pugh分级,将B级肝功能患者随机分为三组(对照组、观察组Ⅰ、观察组Ⅱ),其中对照组患者19例给予肠外营养治疗,观察组Ⅰ21例给予肠内营养治疗,观察组Ⅱ24例给予肠内营养联合肠道益生菌治疗,观察比较三组患者治疗后15、30 d的肝功能相关指标(AST、ALT、TBIL)、营养状况指标(BMI、体脂含量)、凝血时间及并发症发生情况等。结果:治疗15、30 d后,观察组Ⅰ患者的AST、ALT、TBIL及凝血时间与对照组相比,差异均无统计学意义(P>0.05),而治疗15 d后的不良反应发生率明显低于对照组,比较差异有统计学意义(P<0.05)。观察组ⅡAST、ALT、TBIL等肝功能指标在治疗15、30 d后均显著低于对照组和观察组Ⅰ,凝血时间也明显较短,比较差异有统计学意义(P<0.05)。营养支持治疗30 d后,观察组Ⅱ的BMI指数明显高于对照组和观察组Ⅰ,不良反应与其他两组相比明显较低,比较差异有统计学意义(P<0.05)。结论:肠内营养联合肠道益生菌能够明显改善肝硬化失代偿期患者肝功能,缩短凝血时间,降低不良反应发生率,对该类患者的治疗有积极作用。 Objective:To investigate the value and significance of enteral nutrition combined with probiotics in the treatment of patients with liver cirrhosis in loss compensation period.Method:A total of 64 patients with liver cirrhosis in loss compensation period were admitted to the hospital from January to August 2015,according to the admission of bilirubin,albumin,prothrombin time and blood ammonia they were implemented Child-pugh classification,patients with liver function of grade B were randomly divided into three groups(the control group,the observation group Ⅰ,the observation group Ⅱ),19 patients of the control group were treated with parenteral nutrition,21 patients of the observation group Ⅰ were treated with enteral nutrition,24 patients of the observation group Ⅱ were treated with enteral nutrition combined with probiotics,liver function indexes(AST,ALT,TBIL),nutritional status index(BMI,body fat content),coagulation time and complications were observed and compared in three groups after treatment for 15 and 30 days.Result:After treatment for 15 and 30 days,AST,TBIL,ALT and coagulation time between the control group and the observation group Ⅰ were not significantly different(P〈0.05),but the incidence of adverse reactions was significantly lower than that in the control group, the difference was statistically significant(P〈0.05).AST,TBIL and ALT in the observation group Ⅱwere significantly lower than those in the control group and the observation group Ⅰ after treatment for 15 and 30 days,the blood coagulation time was significantly shorter, the difference was statistically significant(P〈0.05).After treatment for 30 days,BMI index of the observation group Ⅱ was significantly higher than that control group and observation group Ⅰ,and the adverse reactions was significantly lower than other two groups, the difference was statistically significant(P〈0.05).Conclusion:Enteral nutrition combined with probiotics can significantly improve liver function pat
