摘要
目的 比较重症亚急性甲状腺炎糖皮质激素药物泼尼松治疗4周短程治疗和8周中程治疗的效果,观察总结4周短程治疗的可行性及优势。方法 筛选出具有可比性的4周短程治疗和8周中程治疗各65例患者设为A、B组,对比两组临床疗效,应用χ2检验进行统计学分析。另对糖皮质激素治疗的315例重症亚急性甲状腺炎患者进行疗程用药的依从率监测。结果 两组患者疗效比较,应用χ2检验进行统计学分析,两组之间总有效率均为100%、无效率均为0,临床疗效差异无统计学意义。其中A组和B组的治愈率、好转率差异有统计学意义（P〈0.05）,A组的好转率高于B组,B组的治愈率高于A组。315例患者的疗程用药的依从率为：4周短程治疗病例均按疗程完成,依从性100%,依从性高;8周中程治疗病例依从性73.79%,依从性低。结论 4周的泼尼松短程治疗,大部分患者可以达到治疗目的,较6~8周中程治疗有疗程短、副作用轻、患者依从性高的优势,更符合2011年发布的糖皮质激素类药物临床应用指导原则,是临床可行并值得推广的一种方法 。
Objective To compare the effects of 4-week short-term treatment and 8-week medium-term treatment by glucocorticoid of prednisone for severe subacute thyroiditis, and to observe and summarize the feasibility and advan- tages of 4-week short-term treatment. Methods 65 patients in comparable 4-week short-term treatment and 8-week medium-term treatment were selected as group A and group B. Clinical curative effects were compared between the two groups, and x^2 test was used for statistical analysis. Monitoring for compliance of drug use in the course of treatment was conducted in 315 patients with severe subacute thyroiditis treated by glucocorticoid. Results In the comparison of curative effects in the two groups of patients, x^2 test was used for statistical analysis, and the total effective rate in the two groups was both 100%, and ineffective rate was both O. There were no clinical and statistical differences. The recovery rate and improvement rate in group A and group B were significantly different （P〈0.05）, with statistical signifi- cance. The improvement rate in group A was higher than that in group B, and the recovery rate in group B was higher than that in group A. The compliance of drug use in course of treatment among 315 patients was： 4-week short-term treatment cases were completed according to the course of treatment, with 100% compliance, indicated high compli- ance; the compliance in 8-week medium-term treatment cases was 73.79%, indicated low compliance. Conclusion Most patients are able to achieve the purpose of treatment in the 4-week short-term treatment by prednisone, and it has advantages of shorter course of treatment, minor adverse effects and high patient compliance compared with 6-8 week medium-term treatment. The short-term treatment also conforms to the guidelines for clinical application of glu- eocorticoids published in 2011, which is a feasible method clinically and worthy of promotion.
