摘要
目的:观察不同药物联合应用对季节性变应性鼻炎(AR)患者症状和生活质量的改善情况。方法:选取96例单用鼻喷激素难以控制症状的中重度季节性AR患者,随机分为布地奈德组(A组)、布地奈德联合非索非那定组(B组)和布地奈德联合孟鲁司特组(C组)进行治疗,分别对治疗2、4周后患者各症状评分、总症状VAS(Uni-VAS)及鼻结膜炎相关生活质量(RQLQ)评分进行评估。结果:B、C组各症状、Uni-VAS及RQLQ评分均低于A组,差异有统计学意义(P<0.05)。B组的鼻痒(用药2周后)、流涕、喷嚏症状评分以及Uni-VAS评分均低于C组(P<0.05),而改善鼻塞方面C组则优于B组(P<0.05),其余症状及RQLQ评分B、C组差异无统计学意义。结论:对于单用鼻喷激素不能完全控制症状的中重度季节性AR患者,联合用药有助于减轻临床症状,提高生活质量。联合用药应根据患者不同症状的严重程度进行个性化选择。
Objective：To observe the effect of combined therapy on seasonal allergic rhinitis（AR）and quality of life.Method：Ninety-six patients with severe seasonal AR unresponsive to intranasal corticosteroids alone were divided randomly into nasal budesonide group（group A）,Nasal Budesonide combined with fexofenadine hydrochloride group（group B）,and budesonide combined with montelukast group（group C）.The symptom scores,UniVAS,and rhinoconjunctivitis quality of life（RQLQ）were evaluated 2and 4weeks after treatment initiation.Result：In the group B and C,the symptom scores,Uni-VAS,and RQLQ were significantly lower than the group A（P〈0.05）.As for nasal itching（after 2weeks）,rhinorrhea and sneezing,the symptom scores and Uni-VAS of group B were significantly lower than that of group C（P〈0.05）.However,compared to group B,the improvement of nasal obstruction in group C was much better（P〈0.05）.There was no significant difference in other symptoms and RQLQ.Conclusion：For patientswith severe seasonal AR unresponsive to intranasal corticosteroids alone,combined therapy can help alleviate clinical symptoms and improve quality of life.The combination of drugs should be individulized based on the severity of symptoms.
