多种抗体检测在甲状腺疾病诊断中的应用价值 被引量:34

摘要 目的探讨促甲状腺激素(TSH)、甲状腺过氧化物酶抗体(TPOAb)、促甲状腺激素受体抗体(TRAb)、甲状腺球蛋白抗体(TG-Ab)等多种抗体检测在甲状腺疾病诊断中的应用价值。方法选取2012年10月~2014年10月在陕西省咸阳市第一人民医院治疗并明确诊断为甲状腺疾病的69例患者为观察对象,同时选取40名健康成年人作为对照组。其中甲状腺疾病患者分为自身免疫性甲状腺疾病组(35例)和非自身免疫性甲状腺疾病组(34例)。运用电化学发光法对三组患者血清中TSH、TPOAb、TRAb、TG-Ab的浓度及阳性率进行检测。结果自身免疫性甲状腺疾病组的TSH、TPOAb、TRAb、TG-Ab浓度和阳性率均显著高于非自身免疫性甲状腺疾病组和对照组(P〈0.05)。TSH、TRAb在自身免疫性甲状腺疾病患者诊断过程中的敏感性、特异性、准确性高于TPOAb和TG-Ab(P〈0.05),各指标(TSH、TPOAb、TRAb、TG-Ab)联合检测自身免疫性甲状腺疾病和非自身免疫性甲状腺疾病的敏感性分别为97.6%、80.1%,特异性分别为98.3%、89.7%,明显高于上述各指标单项检测时的敏感性和特异性。结论TSH、TRAb能很好地检测出甲状腺疾病,值得临床推广。 Objective To investigate the application value of a variety of antibody detection including thyroid stimulating hormone(TSH), thyroid peroxidase antibody(TPOAb), thyroid stimulating hormone receptor antibody(TRAb),thyroglobulin antibodies(TG-Ab) in the diagnosis of thyroid diseases. Methods Sixty nine cases of patients diagnosed as thyroid disease and treated in the First People's Hospital of Xianyang City from October 2012 to October 2014 were selected as the observation group, and 40 cases of healthy adults were selected as the control group. The patients with thyroid disease were divided into autoimmune thyroid disease group(35 cases) and non-autoimmune thyroid disease group(34 cases). Electrochemical luminescence method was used to detect the concentrations and positive rates of the serum TSH, TPOAb, TRAb, TG-Ab of the three groups. Results The concentrations and positive rates of TSH, TPOAb,TRAb, TG-Ab of autoimmune thyroid disease group were significantly higher than those of non-autoimmune disease group and control group(P〈 0.05). The sensibility, specificity, veracity of TSH, TRAb in diagnosis of patients with autoimmune thyroid disease were all higher than those of TPOAb and TG-Ab(P〈 0.05). The sensitivity of the combined detection of all indexes(TSH, TPOAb, TRAb) in diagnosis of the autoimmune thyroid disease and the non-autoimmune thyroid disease was 97.6%, 80.1% respectively, the specificity was 98.3% and 89.7% respectively, which were all higher than those of the single detection of the indexes above. Conclusion TSH, TRAb can be applied to detect the thyroid disease. Therefore, it is worthy of promotion in clinic.
